摘 要:目的分析早期帕金森病(PD)患者多模态MRI成像方式下的影像特征,为PD的发病机制研究提供影像学支撑。方法29例早期帕金森病患者(PD组)和30例健康志愿者(HC组)被纳入研究,分别进行MRI扫描,获取T1加权像(T1WI)、弥散张量成像(DTI)、功能磁共振成像(fMRI)等多个模态图像,经预处理后提取特征值,统计两组差异显著的脑区。结果与HC组比较,PD组多个脑区出现T1体素值或DTI各向异性分数(FA值)显著减少(P<0.05);两组被试有12个脑区的fMRI聚类系数(Cp值)差异有统计学意义(P<0.05);双侧中央后回在T1WI、DTI、fMRI 3种模态下均出现显著性改变(P<0.05)。结论在PD早期,大脑灰质体积已出现显著萎缩,相应脑白质纤维束的完整性破坏,脑功能网络整体结构稳定,但局部结构已遭到破坏;躯体感觉障碍可能是PD早期出现的改变,且症状稳定。Objective To analyze the imaging characteristics in patients with early Parkinson’s disease(PD)with multimodal MRI,and to provide imaging support for the pathogenesis of PD.Methods 29 patients with early Parkinson’s disease(PD group)and 30 healthy controls(HC group)were included in the study.MRI scanning was performed to obtain multiple modal images such as T1 weighted image(T1WI),diffusion tensor imaging(DTI)and functional magnetic resonance imaging(fMRI).After preprocessing,the characteristic values were extracted and the brain regions with significant difference were obtained by statistics.Results The results showed that compared with HC group,T1 voxel value and DTI anisotropy fraction(FA value)in multiple brain regions of PD group were significantly decreased(P<0.05),and the clustering coefficient(Cp value)of fMRI of the two groups were significantly different(P<0.05),bilateral posterior central gyrus showed significant changes in T1WI,DTI and fMRI modes(P<0.05).Conclusion The gray matter volume of the brain has been significantly atrophied in the early stage of PD.With the destruction of the integrity of the corresponding white matter fiber tracts,the overall structure of the brain functional network is stable,but the local structure has been destroyed;somatosensory abnormalities may be the stable and early changes of PD.
