摘  要:目的调查郴州市结核病患者感染的结核分枝杆菌的分子分型特征及流行情况。方法收集来自郴州市第二人民医院分离培养的结核分枝杆菌188株,采用结核分枝杆菌RD105基因位点对北京基因型进行鉴定,同时对菌株进行药敏试验,并选取MIRU-VNTR中的15位点对结核分枝杆菌进行分型鉴定,通过聚合酶链式反应及琼脂糖凝胶电泳对全部基因型数字化结果进行聚类分析。结果188株结核分枝杆菌中北京基因型占79.8%,药敏结果显示耐药株39株(20.74%)。188株结核分枝杆菌可分为3个基因群(Ⅰ、Ⅱ、Ⅲ群),180个基因型,其中Ⅰ群含有144个基因型(80.0%),Ⅱ群含有33个基因型(18.3%),Ⅲ群含有3个基因型(1.7%)。在选定的15个分型位点中分辨力最高的是Mtub21,分辨指数为0.803;其次是MIRU26,分辨指数为0.781;分辨指数最低的是ETRC,分辨指数为0.042。结论郴州市分离的188株结核分枝杆菌具有基因多态性,主要的流行群为Ⅰ群,Mtub21位点、MIRU26位点呈高多态性;郴州市结核病的流行主要是由内源性复燃引起,需重点针对内源性复燃制定结核病的防控策略。Objective To investigate the molecular typing and prevalence of Mycobacterium tuberculosis in Chenzhou city.Methods A total of 188 mycobacterium tuberculosis strains from Chenzhou No.2 People’s Hospital were collected.The Beijing genotype of mycobacterium tuberculosis was identified by loci RD105,and drug sensitivity test was carried out.The 15 loci in MIRU-VNTR were selected to classify mycobacterium tuberculosis,and all the digitized genotypes were analyzed by polymerase chain reaction and agarose gel electrophoresis.Results The Beijing genotype accounted for 79.8%of 188 mycobacterium tuberculosis strains,and 39 strains(20.74%)showed drug resistance.188 mycobacterium tuberculosis strains were divided into three genotype groups(Ⅰ,ⅡandⅢ)with 180 genotypes,of which groupⅠcontained 144 genotypes(80.0%),groupⅡcontained 33 genotypes(18.3%)and groupⅢcontained 3 genotypes(1.7%).Among the 15 loci selected,Mtub21 had the highest resolution,with a resolution index of 0.803.MIRU26 followed with 0.781 resolution index.ETRC has the lowest resolution index of 0.042.Conclusion The 188 mycobacterium tuberculosis strains isolated from Chenzhou city showed genetic polymorphism,and the main epidemic group was groupⅠ.Mtub21 and MIRU26 were highly polymorphic.The epidemic of tuberculosis in Chenzhou city is mainly caused by endogenous relapse,so the prevention and control strategy of tuberculosis should focus on endogenous relapse.
