摘　　要：目的研究子宫内膜异位症(EMs)性不孕患者行辅助生殖助孕的早期自然流产发生率。方法分析比较2016年1月至2021年3月在我院行体外受精(in vitro fertilization,IVF)或单精子卵细胞注射(intracytoplasmic sperm injection,ICSI)助孕的EMs不孕患者(n=198)和输卵管因素不孕患者(n=179)的妊娠结局。结果EMs组自然妊娠的早期流产率显著高于对照组(25.60%vs.14.55%,P<0.05)。辅助生殖助孕后,EMs组受精率(69.22%)及优胚率(51.91%)均低于对照组(79.53%、59.19%,P<0.05)。EMs患者行新鲜或冷冻胚胎移植后早期流产率分别为24.39%、21.31%,均高于对照组(7.14%、4.69%,P<0.05)。结论EMs不孕患者辅助助孕受精率和优胚率降低且流产率升高,提示EMs可通过影响胚胎质量等因素导致不孕不育。ObjectiveTo study the early miscarriage rate of in vitro fertilization in infertile patientswith endometriosis(EMs).MethodsPregnancy outcomes of 198 infertile patients with EMs and 179 infertilitypatients with tubal problems treated with IVF or ICSI in our hospital from January 2016 to March 2021 were retro-spectively investigated.ResultsThe early miscarriage rate of natural pregnancy in the endometriosis group wassignificantly higher than that in the control group(25.60%vs.14.55%,P<0.05).After IVF or ICSI,the fertiliza-tion rate and high-quality embryo rate in the endometriosis group were significantly lower than those in the controlgroup,respectively(69.22%vs.79.53%;51.91%vs.59.19%,P<0.05).The early miscarriage rate after the im-plantation of fresh or frozen embryos in the EMS patients was significantly higher than that in the control group,respectively(24.39%vs.7.14%,21.31%vs.4.69%,P<0.05).ConclusionThe fertilization rate and high-qualityembryo rate are lower and the early miscarriage rate is higher in the EMS patients,indicating that endometriosismay lead to infertility by impairing the embryo quality and other factors.
