摘 要:目的与甲状腺超声检查对比,评估使用8通道甲状腺专用表面线圈进行磁共振检查的价值。方法前瞻性对49名健康志愿者行甲状腺磁共振和超声检查,磁共振检查采用8通道甲状腺专用表面线圈。以超声检查结果为金标准,分别从结节数目、大小、位置、性质多方面评估磁共振对甲状腺结节的检查价值,将甲状腺超声检出的结节分为长径≤3 mm、3 mm<长径<10 mm、长径≥10 mm三组,分析磁共振对不同组别甲状腺结节的检出效能。结果甲状腺超声检查,49名志愿者中共有23名检出30个结节,MR检出其中21名的27个结节,结节检出率为90%。MR分别检出超声所检出长径≤3 mm、3 mm<长径<10 mm、长径≥10 mm三组结节11个、15个、4个中的9个(81.82%)、14个(93.33%)、4个(100%)。对于甲状腺超声和磁共振均检出的27个结节,甲状腺超声检查长径、短径平均分别为6.17 mm、3.76 mm,MR检查长径、短径平均分别为5.58 mm、3.91 mm,两种检查方式长径和短径的差异均没有统计学意义;在结节性质方面,磁共振检出的27个结节中有26个性质和超声是一致的,其余1个结节,超声诊断为囊实性,而磁共振诊断为囊性。结论与甲状腺超声检查相比,使用8通道甲状腺专用表面线圈进行磁共振扫描,在结节数目、大小、位置以及性质方面均具有明显的一致性。Objective To evaluate the value of a 8-channel MRI surface coil exclusively designed for thyroid gland in thyroid imaging by comparing that of ultrasound examination.Methods In this prospective study,49 healthy volunteers consecutively underwent thyroid MRI scan using an 8-channel thyroid surface coil and ultrasound examination.With the ultrasound findings as the gold standard,the value of MR using the specific surface coil in the detection and depiction of thyroid nodules was evaluated in terms of the detection rate,size measurement,localization and characterization depiction of the nodules with the ultrasound findings as reference.Based on the long diameter,the thyroid nodules detected by thyroid ultrasound were divided into three groups(≤3 mm,3-10 mm,≥10 mm),detection efficacy by the MRI was analyzed in each groups of thyroid nodules.Results 30 nodules in 23 volunteers(23/49)were detected by ultrasound examination,in which a total of 27 nodules were successfully detectedin21 volunteers(21/49)by thyroid MRI with the nodule detection rate of 90%.Among the nodules detected by ultrasound examination in the three nodule groups(long diameter≤3 mm,3-10 mm and≥10 mm),9(9/11),14(14/15)and 4(4/4)were successfully detected by MRI correspondingly.For 27 nodules detected both by thyroid ultrasound and MR,no significant difference was found regarding the average long diameter(6.17 vs 5.58 mm)and the average short diameter(3.76 vs 3.91 mm).As for nodule characterization,consistency was found in 26 of the 27 nodules between MRI and ultrasound with 1 cystic solid nodules characterized by the ultrasound classified as cystic nodules by MRI.Conclusion In comparison to the performance of thyroid ultrasound examination,thyroid MR examination using the 8-channel surface coil exclusively designed for thyroid gland could show higher consistency in the nodule detection,size measurement,localization,and nodule characterization.
