老年全膝关节置换患者疾病感知与居家康复依从性的关系研究

摘　　要：目的了解老年全膝关节置换患者疾病感知、居家康复训练依从性现状及关系,并分析居家康复训练依从性的影响因素。方法采用一般资料调查表、简易疾病感知问卷、居家康复训练依从性量表对老年全膝关节置换患者进行调查。结果老年全膝关节置换患者疾病感知得分为(44.17±4.66)分,居家康复训练依从性得分为(30.02±4.35)分,疾病感知总分和居家康复训练依从性总分及各维度得分呈负相关(P<0.01或P<0.05),回归分析显示,婚姻状况、膝关节疼痛时间和疾病感知是老年全膝关节置换患者居家康复训练依从性的影响因素(P<0.01或P<0.05)。结论老年全膝关节置换患者的居家康复训练依从性有待进一步提高,医护人员应重视患者的婚姻状况、膝关节疼痛时间和疾病感知,采取有效的干预措施促进更积极的疾病感知水平,提高患者的康复训练依从性,改善术后康复。Objective To investigate the status and relationship of illness perception and home-based rehabilitation adherence in elderly patients with total knee arthroplasty,and to analyze the influencing factors of home-based rehabilitation adherence.Methods General information questionnaire,brief illness perception questionnaire and home-based rehabilitation adherence scale were used to investigate the elderly patients with total knee arthroplasty.Results The illness perception score of elderly patients with total knee arthroplasty was(44.17±4.66),and the total score of home-based rehabilitation adherence was(30.02±4.35).The total score of illness perception was negatively correlated with the total score of home-based rehabilitation adherence and the scores of all dimensions(P<0.01 or P<0.05).The regression analysis showed that marital status,the duration of knee pain and illness perception were the influencing factors of rehabilitation adherence that occupied the home rehabilitation training(P<0.01 or P<0.05).Conclusion The home-based rehabilitation adherence in elderly patients with knee arthroplasty remains to be further improved,medical staff should attach importance to the patients'marital status,duration of knee pain,and illness perception,adopt effective interventions to promote a more positive disease awareness level,improve the patient's rehabilitation adherence,improve postoperative rehabilitation.
