“多三角瓣”去皮法在下睑袋切除术中的应用

摘　　要：目的:介绍一种在经皮肤入路下睑袋切除术中,可安全切除多余皮肤的手术方法。方法:在常规进行下睑袋切除后,将下睑皮瓣向上提拉,在外眦转折点处垂直剪开皮瓣至去皮量,形成内、外两个三角皮瓣,将剪开的分叉处固定至外眦处眶骨深层至骨膜。向上提拉皮瓣,首先去除外侧三角形皮肤瓣,然后由外向内以三角形为单位分次切除内侧多余皮肤,逐步修剪至与切口上缘对合平齐。结果:本组100例患者术后下睑袋矫正效果明显,均未出现下睑外翻情况。患者眶周老化特征改善显著,外形自然,瘢痕不明显。结论:“多三角瓣”去皮法在下睑袋切除术中具有安全、有效、可行性好等优点。Objective To introduce a safe method of removing redundant skin in transcutaneous lower eyelid blepharoplasty.Methods After removal of the prominent orbital septal fat,the skin muscle flap was lifted and cut vertically,at lateral canthus,into two triangular flaps.The incised bifurcated point of the skin muscle flap was suspended and fixed to periosteum of the orbital bone at lateral canthus.The lateral triangular skin flap was excised,then the medial triangular skin flap and redundant skin were excised in triangles,laterally to medially,until the lower edge of the incision matched the upper edge.Results All 100 patients got satisfactory results.Lower eyelid bag was improved obviously,with natural appearance and no obvious scar.Conclusion It is safe,effective and feasible to use“continuous small triangle”flap excision in lower eyelid blepharoplasty.
