L形膨体聚四氟乙烯假体联合少量耳软骨在鼻整形术中的应用

摘　　要：目的:观察L形膨体聚四氟乙烯(Expanded polytetrafluoroethylene,e-PTFE)假体联合单侧耳甲腔软骨在鼻整形术中的应用效果。方法:采用Goodman切口开放入路,将L形膨体聚四氟乙烯假体作为支架抬高延长鼻尖,将假体的鼻小柱部分插入鼻翼软骨内侧脚间腔隙内形成悬浮型支撑杆移植物。松解提拉双侧鼻翼软骨,将双侧鼻翼软骨穹窿部缝合固定于膨体鼻尖部顶端。切取耳甲腔软骨雕刻成盾牌及长椭圆形帽状移植物固定于假体前顶端及鼻翼软骨穹窿部尾侧缘。结果:本组66例就医者术后鼻长度较术前增加3~8mm,平均4mm,鼻尖突出度增加3~7mm,平均4mm,鼻尖形态满意。1例出现术后感染。结论:单侧耳甲腔软骨联合L形膨体聚四氟乙烯假体用于开放入路轻中度短鼻、低鼻整形,方法简单可靠,效果良好,适合东亚人鼻整形。Objective To study an method combined L-shaped e-PTFE implantation and a small amount of auricular cartilage graft in rhinoplasty Methods Used Goodman incision open approach,L-shaped e-PTFE prosthesis was used as a stent to raise and extend the nasal tip,and the nasal column of the prosthesis was inserted into the space between the medial feet of alar cartilage to form a suspended support rod graft.Release and pull bilateral alar cartilage,suture and fix bilateral alar cartilage fornix on the top of expanded nose tip.The concha cartilage was cut and carved into a shield and a long oval cap graft was fixed on the front top of the prosthesis and the caudal edge of the alar cartilage fornix.Results The nasal length of 66 patients increased by 3-8mm,with an average of 4mm,and the protrusion of nasal tip increased by 3-7mm,with an average of 4mm.The shape of nasal tip was satisfactory.Postoperative infection occurred in one case.Conclusion Unilateral concha cartilage combined with L-shaped e-PTFE prosthesis is used for open approach,mild to moderate short nose and low nose plastic surgery.The method is simple,reliable and effective.It is suitable for East Asian nose plastic surgery.
