5-氨基酮戊酸光动力联合低剂量异维A酸治疗重度痤疮疗效及安全性分析

摘　　要：目的:观察5-氨基酮戊酸光动力联合低剂量异维A酸胶囊治疗重度痤疮的安全性及疗效。方法:将入选的90例患者按就诊顺序分为异维A酸组、光动力组及联合组,每组30例。异维A酸组予异维A酸胶囊0.25mg/kg·d,疗程12周;光动力组予5-氨基酮戊酸光动力治疗,每周1次,共4次;联合组予异维A酸胶囊0.25mg/kg·d,疗程12周,联合光动力4次治疗。比较三组治疗后4周、12周的有效率及不良反应情况,评判临床疗效及安全性。结果:治疗4周后,光动力组与联合组有效率均高于异维A酸组(均P<0.05);治疗12周后,联合组有效率高于光动力组及异维A酸组(均P<0.05)。治疗过程中,红斑、疼痛在光动力组及联合组中发生率高于异维A酸组(均P<0.05),唇炎在异维A酸组及联合组发生率高于光动力组(均P<0.05),干燥脱屑、血液学、湿疹及色素沉着的发生率三组比较差异均无统计学意义(均P>0.05)。结论:5-氨基酮戊酸光动力联合低剂量异维A酸递减疗法治疗重度痤疮疗效确切,起效快、远期疗效佳,且不良反应轻,值得临床应用推广。Objective To observe the safety and effectiveness of 5-aminolevulinic acid photodynamic combined with low-dose isotretinoin capsules in the treatment of severe acne.Methods The selected 90 patients were divided into isotretinoin group,photodynamic group and combination group according to the order of treatment,with 30 cases in each group.The isotretinoin group was given isotretinoin capsules 0.25 mg/kg•d for 12 weeks.The photodynamic group was treated with 5-aminolevulinic acid photodynamic therapy once a week for a total of 4 times.The combined group was given Isotretinoin capsules 0.25 mg/kg•d for 12 weeks combined with photodynamic therapy 4 times.The effective rates and adverse reactions of the 3 groups after 4 weeks and 12 weeks of treatment were compared to evaluate the clinical efficacy and safety.Results After 4 weeks of treatment,the photodynamic group and the combination group had higher effective rates than the isotretinoin group(all P<0.05).After 12 weeks of treatment,the effective rate of the combined group was higher than that of the photodynamic group and the isotretinoin group(all P<0.05).During of treatment,the incidence of erythema and pain in the photodynamic and combination groups was higher than that in the isotretinoin group(all P<0.05).The incidence of cheilitis in the isotretinoin group and the combination group was higher than that in the photodynamic group(all P<0.05).There was no statistical difference in the incidence of dry desquamation,hematology,eczema and pigmentation among the three groups(all P>0.05).Conclusion 5-Aminolevulinic acid photodynamic combined with low-dose isotretinoin diminishing therapy is effective in the treatment of severe acne,with quick onset,good long-term effect and mild adverse reactions,and it is worthy of clinical application and promotion.
