摘  要:目的研究冠状动脉粥样硬化性心脏病(coronary heartdisease,CHD)患者血清白细胞介素(interleukin,IL)-35、成纤维细胞生长因子-23(fibroblast growth factor-23,FGF-23)、胎球蛋白A浓度及其与冠状动脉狭窄的相关性。方法选取2018年2月至2019年2月兰州大学第一医院收治的102例CHD患者为CHD组,以同期本院50名健康查体者为对照组。采用Gensini评分评价CHD组患者冠状动脉狭窄严重程度。应用酶联免疫吸附法检测各组患者血清IL-35、FGF-23及胎球蛋白A浓度。采用Pearson线性相关分析分析各指标与冠状动脉狭窄严重程度的相关性。采用单因素及多因素Logistic回归分析分析冠状动脉狭窄严重程度的危险因素。结果与对照组相比,CHD组血清IL-35及胎球蛋白A浓度明显较低,而FGF-23浓度明显较高,差异有统计学意义(均P<0.05)。CHD组Gensini评分与血清FGF-23呈显著正相关,而与血清IL-35、胎球蛋白A呈显著负相关(均P<0.05)。冠状动脉狭窄严重程度与糖尿病史、血清FGF-23、IL-35及胎球蛋白A浓度有关(均P<0.05)。多因素Logistic回归分析显示,糖尿病史、血清FGF-23、IL-35及胎球蛋白A是冠状动脉狭窄严重程度的危险因素(P<0.05)。结论冠心病患者血清FGF-23浓度升高,而IL-35、胎球蛋白A浓度降低,三者在CHD的发生、发展过程起到重要作用,是冠状动脉狭窄严重程度的危险因素。Objectives To study the serum concentrations of interleukin-35(IL-35),fibroblast growth factor-23(FGF-23)and fetuin-A in patients with coronary heart disease(CHD)and their correlations with coronary artery stenosis.Methods Totally 102 patients with CHD admitted to The First Hospital of Lanzhou University from February 2018 to February 2019 were selected as CHD group,and 50 patients who underwent health examinations during the same period were selected as control group.The degree of coronary artery stenosis in patients with CHD was evaluated by Gensini score.The serum concentrations of IL-35,FGF-23 and fetuin-A were detected by enzyme-linked immunosorbent assay.The correlation of related indicators and coronary stenosis was analyzed by Pearson linear correlation analysis.The risk factors for the severity of coronary stenosis was analyzed by multivariate Logistic regression analysis.Results Compared with control group,serum concentrations of IL-35 and fetuin-A were significantly lower in CHD group,while concentration of FGF-23 was significantly higher(both P<0.05).Gensini score in CHD group significantly positively correlated with serum concentration of FGF-23,and significantly negatively correlated with serum concentrations of IL-35 and fetuin-A(all P<0.05).The severity of coronary stenosis related to the history of diabetes,serum concentration of FGF-23,IL-35 and fetuin-A(all P<0.05).Multivariate Logistic regression analysis showed that history of diabetes,serum concentrations of FGF-23,IL-35,and fetuin-A were risk factors for the severity of coronary artery stenosis(P<0.05).Conclusions Serum concentration of FGF-23 increases in patients with CHD,while concentrations of IL-35 and fetuin-A decrease.These three factors play an important role in the development of CHD and are risk factors for the severity of coronary artery stenosis.
