推拿联合催乳汤与心理干预对剖宫产后产妇功能恢复及血清泌乳素、催产素水平的影响

摘　　要：目的研究中医推拿联合催乳汤与心理干预对剖宫产后产妇功能恢复及血清泌乳素、催产素水平的影响。方法选取86例接受剖宫产分娩的产妇作为研究对象,选取时间为2018年5月—2020年6月,分组方法依据随机数字表法,将所纳入产妇分为对照组和研究组,每组43例。对照组产妇于产后第2天口服催乳汤,研究组在其基础上于产后6 h内接受中医推拿,均治疗7 d时间,治疗期间均接受心理干预。比较2组产妇产后功能恢复情况以及母乳喂养情况;产后72 h、7 d血清泌乳素、催产素水平变化。结果治疗后,研究组产妇产后24 h出血量与对照组相比明显减少;研究组子宫三径之和明显小于对照组;研究组胃肠功能恢复时间短于对照组;研究组产妇产后72 h、7 d血清泌乳素、催产素水平与对照组相比均明显升高;研究组产妇泌乳量满足率(83.72%)、母乳喂养率(90.70%)、新生儿睡眠正常率(90.70%)均高于对照组(48.84%、60.47%、62.79%)(均P<0.05)。结论中医推拿联合催乳汤与心理干预对剖宫产后产妇功能恢复有显著提升作用,同时有效提高产妇母乳喂养成功率,改善新生儿睡眠质量,显著升高产妇血清泌乳素、催产素水平,提高治疗效果。Objective To study the effect of traditional Chinese medicine massage combined with Cuiru decoction and psychological intervention on the functional recovery and serum prolactin and oxytocin levels of puerpera after cesarean section.Methods 86 cases of cesarean delivery in department of obstetrics and gynecology of our hospital were selected as the research objects,and the time was from May 2018 to June 2020.According to the random number table method,the included women were divided into two groups,named as control group and study group,with 43 cases in each group.The control group was given oral Cuiru decoction on the second day after delivery,and the study group was given traditional Chinese medicine massage within 6 hours after delivery on the basis of Cuiru decoction.All the patients were treated for 7 days and received psychological intervention during the treatment.The recovery of postpartum function and breastfeeding,and the changes of serum prolactin and oxytocin levels 72 h and 7 d after delivery were compared between the two groups.Results The analysis after treatment found that the amount of postpartum hemorrhage in the study group was significantly reduced compared with the control group.The sum of the three uterine diameters in the study group was significantly less than the control group.The recovery time of gastrointestinal function in the study group was shorter than that in the control group.The levels of serum prolactin and oxytocin in the study group were significantly higher than those in the control group at 72 h and 7 d postpartum,and the levels of prolactin and oxytocin in the study group were significantly higher.The satisfaction rate of lactation(83.72%),breastfeeding rate(90.70%)and normal sleep rate of newborns(90.70%)in the study group were higher than those in the control group(48.84%,60.47%and 62.79%)(all P<0.05).Conclusion Traditional Chinese medicine massage combined with Cuiru decoction and psychological intervention can significantly improve the functional recovery of postpartum
