制霉菌素联合伊曲康唑治疗念珠菌性阴道炎的临床疗效及其安全性

摘　　要：目的探讨制霉菌素联合伊曲康唑治疗念珠菌性阴道炎的临床疗效及其安全性。方法选取兴国县皮肤病性病防治所2018年6月—2020年6月收治的念珠菌性阴道炎患者86例,按照随机数字表法分为对照组(n=43)与试验组(n=43)。对照组采用制霉菌素治疗,试验组采用制霉菌素联合伊曲康唑治疗。2组均连续治疗14 d。比较2组临床疗效,治疗前后临床症状(包括白带、瘙痒、烧灼感)积分、炎性因子〔白介素4(IL-4)、白介素8(IL-8)〕,不良反应发生率、复发率。结果试验组总有效率高于对照组(P<0.05)。治疗前,2组白带、瘙痒、灼烧感积分比较,差异无统计学意义(P>0.05);治疗后,试验组白带、瘙痒、灼烧感积分低于对照组(P<0.05)。治疗前,2组IL-4、IL-8比较,差异无统计学意义(P>0.05);治疗后,试验组IL-4、IL-8低于对照组(P<0.05)。2组不良反应发生率、复发率比较,差异无统计学意义(P>0.05)。结论念珠菌性阴道炎患者应用制霉菌素联合伊曲康唑治疗可提高疗效,改善机体炎症状态,缓解临床症状,且安全性较高。Objective To investigate the clinical effect and the safety of nystatin combined with itraconazole in the treatment of candidal vaginitis.Methods A total of 86 cases of patients with candidal vaginitis were selected from June 2018 to June 2020 in Institute of Dermatology and Venereology,Xingguo County,which were divided into control group(n=43)and experimental group(n=43)according to random number table method.The control group was treated with nystatin,while the experimental group was treated with nystatin combined with itraconazole.Both groups were treated continuously for 14 days.Clinical efficacy,scores of clinical symptoms and signs(including leucorrhea,itching and burning sensation),IL-4 and IL-8 before and after treatment,incidence of adverse reactions and recurrence rate were compared between the two groups.Results The total effective rate of the experimental group was higher than that of the control group(P<0.05).Before treatment,there was no significant difference in scores of leucorrhea,itching and burning between the two groups(P>0.05);after treatment,the scores of leucorrhea,itching and burning in the experimental group were lower than those in the control group(P<0.05).Before treatment,there was no significant difference in IL-4 and IL-8 between the two groups(P>0.05);after treatment,IL-4 and IL-8 in the experimental group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions and recurrence rate between the two groups(P>0.05).Conclusion The application of nystatin combined with itraconazole in the treatment of candidal vaginitis can improve the curative effect,improve the inflammatory state of the body and relieve the clinical symptoms,and with high safety.
