复方甘草酸苷注射液辅助治疗小儿过敏性紫癜肾炎的疗效及对细胞因子的影响

摘　　要：目的探讨复方甘草酸苷注射液辅助治疗小儿过敏性紫癜肾炎的疗效及对细胞因子的影响。方法选取60例过敏性紫癜肾炎患儿,按抓阄法随机将其平均分为甲、乙两组,各30例。甲组进行常规治疗,乙组在甲组基础上辅以复方甘草酸苷注射液治疗,两组均治疗4周后,观察临床疗效,比较尿蛋白定量、尿红细胞、血清肌酐、尿微量白蛋白(mAlb)指标变化;细胞因子[白细胞介素-4(IL-4)、γ-干扰素(IFN-γ)]水平变化。结果治疗4周后,乙组总有效率90.00%显著高于甲组66.67%(P<0.05);治疗后,两组患儿尿蛋白定量、尿红细胞、血清肌酐均明显下降,尿微量白蛋白(mAlb)明显增高(P<0.05),且乙组优于甲组(P<0.05);治疗后,两组患儿IL-4水平明显降低,IFN-γ水平明显增高(P<0.05),且乙组优于甲组(P<0.05)。结论复方甘草酸苷注射液辅助治疗小儿过敏性紫癜肾炎临床疗效好,可增强肾功能,调节细胞因子水平。Objective To investigate the therapeutic effect of compound glycyrrhizin injection in adjuvant treatment of children with Henoch-Schonlein purpura nephritis(HSPN)and the influence on cytokines.Methods A total of 60 children with HSPN were randomized into two groupA and group B,30 cases in each group.Group A was given conventional treatment,and group B was treated with compound glycyrrhizin injection on the basis of conventional treatment.After 4 weeks of treatment,clinical effects were observed.Changes in urine protein quantification,urine erythrocyte,serum creatinine,urine microalbumin(MAlb)and cytokines[interleukin-4(IL-4),γ-interferon(IFN-γ)]were compared between groups.Results After 4 weeks of treatment,the total response rate of group B(90.00%)was significantly higher than that of group A(66.67%)(P<0.05).After treatment,urinary protein quantification,urine erythrocyteand serum creatinine of both groups were significantly decreased,and urine microalbumin(MAlb)significantly increased(P<0.05).Besides,the above indexes in group B were better than group A(P<0.05).IL-4 levels in both groups significantly decreased,andIFN-γlevels significantly increased(P<0.05),which were better in group B than group A(P<0.05).Conclusion Compound glycyrrhizin injection is effective in adjuvant treatment of children with HSPN,which can enhance renal function and regulate cytokine levels.
