摘  要:目的:研究体外膈肌起搏器结合肺康复治疗对慢性阻塞性肺气肿肺功能和生活质量的影响。方法:62例患者随机分为实验组(肺康复+体外膈肌起搏器)和对照组(常规康复)。所有患者均接受慢阻肺治疗,感染、扩张支气管、祛痰、吸氧、机械通气等常规治疗。4周后对所有患者进行干预前后肺功能、血气指标、6分钟步行距离(6WMD)和生活质量(SF-36)评估。结果:两组患者干预后FEV1、FVC、FEV1/FVC、FEV1%、PaO2和PaCO2和干预前对比,统计学有明显差异(P<0.05)。干预后两组患者6分钟步行距离(6WMD)和生活质量(SF-36)和干预前对比有统计学差异(P<0.05)。结论:肺康复联合体外膈肌起搏器治疗可以更好地改善慢性阻塞性肺气肿患者的肺功能指标,血气分析指标以及生活质量和活动能力。Objective:To study the effect of pulmonary rehabilitation plus external diaphragmatic pacemaker for Chronic Obstructive Pulmonary Disease.Methods:62 patients were randomized into experimental groups(pulmonary rehabilitation+in vitro phrenic pacemaker) and control groups(routine rehabilitation).All patients receive COPD treatment,infection,bronchial dilation,expectorant,oxygen inhalation,mechanical ventilation and other routine treatment.After 4 weeks,lung function,blood gas indicators,6-minute walking distance(6 WMD),and quality of life(SF-3 6) were evaluated for all patients before and after the intervention.Results:Statistical differences were between FEV1,FVC,FEV1/FVC,FEV1%,Pa02 and PaCO2 and pre-intervention(P<0.05).There were statistical differences between the two groups with 6-minute walk distance(6 WMD) and quality of life(SF-36) and pre-intervention comparisons(P<0.05).Conclusions;Pulmonary rehabilitation consortium external diaphragm pacemaker therapy can better improve the pulmonary function indicators,blood gas analysis indicators,quality of life and activity ability in patients with chronic obstructive emphysema.
