摘　　要：目的分析粉尘螨滴剂舌下含服特异性免疫治疗在小儿过敏性哮喘患儿中的应用效果。方法选择2017年1月—2018年1月本院收治的小儿过敏性哮喘患儿90例开展本研究,根据治理方式不同分为参照组(45例)与试验组(45例),参照组给予临床哮喘规范化治疗,试验组在此基础上给予粉尘螨滴剂舌下特异性免疫治疗,每天1次,根据患儿病情逐渐增加药物剂量直至维持量,两组疗程均为半年,之后分别对比两组患儿的哮喘控制情况、肺功能改善情况、不良反应发生率,分别检测患儿治疗前后的血清总IgE、白细胞介素2(IL-2)、白细胞介素4(IL-4)等指标水平变化情况。结果治疗后,试验组患儿哮喘总控制率为97.78%,明显高于参照组患儿的哮喘总控制率68.89%,对比差异有统计学意义(P<0.05);试验组患儿的用力呼气量(FVC)、呼气峰值流速(PEF)及FEV%等肺功能相关指标显著升高,且明显高于参照组,差异有统计学意义(P<0.05);试验组的血清总IgE、IL-4较治疗前显著降低,而IL-2较治疗前显著升高,与参照组对比,差异有统计学意义(P<0.05);两组患儿均未出现任何明显的不良反应。结论采取粉尘螨滴剂舌下特异性免疫治疗儿童过敏性哮喘,能显著控制患儿哮喘发作,改善患儿的肺功能,疗效确切,安全性高。Objective To analyze the effect of sublingual specific immunotherapy with dust mite drops in children with allergic asthma.Methods 90 children with allergic asthma treated in our hospital from January 2017 to January 2018 were selected for this study.According to different treatment methods,they were divided into reference group(45 cases)and experimental group(45 cases).The reference group was given the standardized treatment of clinical asthma,and the experimental group was given sublingual specific immunodesensitization therapy with dust mite drops,once a day,and the drugs were gradually increased according to the condition of the children After the treatment,the asthma control,lung function improvement and the incidence of adverse reactions were compared between the two groups.The serum total IgE,interleukin-2(IL-2),interleukin-4(IL-4)were detected before and after treatment.Results After treatment,the total asthma control rate of the experimental group was 97.78%,which was significantly higher than that of the control group(68.89%),with statistical significance(P<0.05);the lung function related indexes such as forced expiratory volume(FVC),peak expiratory flow rate(PEF)and FEV%in the experimental group were significantly higher than those in the control group,P<0.05;the total serum IgE and I in the experimental group were significantly higher than those in the control group(P<0.05)L-4 was significantly lower than that before treatment,while IL-2 was significantly higher than that before treatment,P<0.05;there were any obvious adverse reactions in both groups.Conclusion Sublingual specific immunodesensitization with Dermatophagoides Farinae Drops in the treatment of children with allergic asthma can significantly control the attack of asthma and improve the lung function of children with definite curative effect and high safety.
