基于DEA-Malmquist指数法对某三级甲等综合性公立医院临床科室运行效率的评价研究

摘　　要：目的分析2016年—2020年厦门市某三级甲等综合性公立医院临床科室运行效率,为相关部门合理配置医疗卫生资源,提高医院运行效率提供决策参考依据。方法采用DEA模型对2020年23个临床科室的运行效率进行静态分析,采用Malmquist指数对2016年—2020年各科室的运行效率进行动态分析。结果2020年23个临床科室的综合技术效率均值为0.809,其中7个科室的综合技术效率达到DEA有效,占比30.4%;2016年—2020年23个临床科室的全要素生产率指数均值为0.942,其中7个科室的全要素生产率指数均值大于1,占比30.4%。结论2020年该医院临床科室运行效率不高,不同程度存在着规模过大或产出不足、资源利用率较低等情况;2016年—2020年23个临床科室的全要素生产率指数均值为0.942,处于“无效”状态,但总体呈波动性上升趋势;各临床科室效率的变化受到纯技术效率和规模效率共同影响,但纯技术效率影响较大。Objective To analyze the operating efficiency of clinical departments in a class A tertiary general public hospital in Xiamen from 2016 to 2020 to provide reference for decision-making to rationally allocate medical and health resources and improve the efficiency of hospital operation.Methods The DEA model was used to statically analyze the operating efficiency of 23 clinical departments in 2020,and the Malmquist index was used to dynamically analyze the operating efficiency of each department from 2016 to 2020.Results In 2020,the average comprehensive technical efficiency of 23 clinical departments was 0.809,of which 7 departments reached DEA effectiveness,accounting for 30.4%;the average total factor productivity index of the 23 clinical departments from 2016 to 2020 was 0.942,of which 7 departments had a value greater than 1,accounting for 30.4%.Conclusion In 2020,the operating efficiency of clinical departments in this hospital was not high,and there was overcapacity,insufficient output,and low resource utilization to various degrees;the average total factor productivity index of the 23 clinical departments from 2016 to 2020 was 0.942,which was in an"ineffective"status,but showed an overall upward trend;the changes in the efficiency of various clinical departments are affected by the combined action of technical and scale efficiency,but the technical efficiency had a greater impact.
