针刺联合颈椎牵引治疗椎基底动脉缺血性眩晕的临床效果

摘　　要：目的探讨针刺联合颈椎牵引治疗椎基底动脉缺血性眩晕的临床效果。方法选取2018年12月至2019年6月沈阳市骨科医院承基医院神经内科收治的100例椎基底动脉缺血性眩晕患者作为研究对象,采用随机数字表法分为颈椎牵引组(50例)与联合治疗组(50例)。颈椎牵引组采用颈椎牵引方法,联合组采用颈椎牵引法联合针刺治疗法,治疗时间为20 d。比较两组的头晕目眩、视物旋转、头痛、耳鸣、恶心呕吐证候积分、椎-基底动脉系统缺血性神经功能障碍评分、ABCD2评分、椎基底动脉血管搏动指数(PI)、收缩期最大流速(VS)及治疗总有效率。结果治疗后,联合治疗组头晕目眩、视物旋转、头痛、耳鸣、恶心呕吐证候积分、神经功能障碍评分、ABCD2评分、PI均低于颈椎牵引组,差异有统计学意义(P<0.05)。治疗后,联合治疗组的VS高于颈椎牵引组,差异有统计学意义(P<0.05)。联合治疗组治疗总有效率高于颈椎牵引组,差异有统计学意义(P<0.05)。结论针刺联合颈椎牵引治疗具有提高血流速度、改善血液循环,发挥降低血脂、缓解血管痉挛、提高神经自主能力等功效,提高治愈率。Objective To investigate the clinical efficacy of acupuncture combined with cervical traction in the treatment of vertebrobasilar ischemic vertigo.Methods A total of 100 patients with vertebrobasilar ischemic vertigo admitted to the Department of Neurology in the Chengji Hospital of Shenyang Orthopedic Hospital from December 2018 to June 2019 were selected.They were divided into the cervical traction group(50 patients)and the combined treatment group(50 patients)according to the random number table method.The cervical traction group was given cervical traction,and the combined treatment group was given cervical traction combined with acupuncture.The treatment duration was 20 days.The scores of vertigo,visual rotation,headache,tinnitus,nausea,vomiting,vertebrobasilar system ischemic neurological dysfunction,ABCD2,vertebrobasilar pulsitility index(PI),velocity systolic(VS)and total effective rate were compared between the two groups.Results After treatment,the scores of vertigo,visual rotation,headache,tinnitus,nausea,vomiting,neurological dysfunction,ABCD2,and PI in the combined treatment group were lower than those in the cervical traction group,with statistically significant differences(P<0.05).After treatment,the score of VS in the combined treatment group was higher than that in the cervical traction group,with statistically significant difference(P<0.05).The total effective rate in the combined treatment group was higher than that in the cervical traction group,with statistically significant difference(P<0.05).Conclusion Acupuncture combined with cervical traction can increase blood flow speed,improve blood circulation,reduce blood lipids,relieve vasospasm,improve neuro-autonomy,and increase cure rate.
