半夏厚朴汤加减联合奥美拉唑对痰气郁结型胃食管反流患者的临床疗效

摘　　要：目的探讨半夏厚朴汤加减联合奥美拉唑对痰气郁结型胃食管反流患者的临床疗效。方法84例患者随机分为对照组和观察组,每组42例,对照组给予奥美拉唑,观察组在对照组基础上加半夏厚朴汤,疗程3周。检测临床疗效、中医证候评分、DCI、GerdQ评分、胃肠激素(胃促生长素、胃泌素)、肠道菌群(肠球菌、肠杆菌、双歧杆菌、乳酸杆菌)变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组中医证候评分、GerdQ评分、肠球菌、肠杆菌降低(P<0.05),DCI、胃肠激素、双歧杆菌、乳酸杆菌升高(P<0.05),以观察组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论半夏厚朴汤加减联合奥美拉唑可安全有效地改善痰气郁结型胃食管反流患者临床症状,提高疗效,其作用机制可能与调节血清胃肠激素水平、改善肠道菌群分布有关。AIM To explore the clinical effects of modified Banxia Houpu Decoction combined with omeprazole on patients with gastroesophageal reflux due to Qi-Depression and Phlegm-Stagnation.METHODS Eighty-four patients were randomly assigned into control group(42 cases)for 3-week administration of omeprazole,and observation group(42 cases)for 3-week administration of both modified Banxia Houpu Decoction and omeprazole.The changes in clinical efficacy,TCM syndrome scores,DCI,GerdQ score,gastrointestinal hormones(ghrelin,gastrin),intestinal flora(enterococcus,enterobacteriaceae,bifidobacterium,lactobacillus)were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome scores,GerdQ score,enterococcus and enterobacteriaceae(P<0.05),and increased DCI,gastrointestinal hormones,bifidobacterium and lactobacillus(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with gastroesophageal reflux due to Qi-Depression and Phlegm-Stagnation,modified Banxia Houpu Decoction combined with omeprazole can safely and effectively improve clinical symptoms and enhance efficacy,whose mechanism may contribute to the regulation of serum gastrointestinal hormone levels and improvement of intestinal flora distribution.
