腹腔镜巨脾切除加断流术治疗肝硬化门静脉高压症的效果

摘　　要：目的研究肝硬化门静脉高压症患者治疗过程中应用腹腔镜巨脾切除联合断流术的临床效果。方法54例肝硬化门静脉高压症患者,根据治疗方法的不同分为对照组与研究组,每组27例。对照组患者采用传统开腹手术进行治疗,研究组采用腹腔镜巨脾切除联合断流术进行治疗。对比两组临床指标、住院情况、并发症发生率。结果研究组手术时间、肛门排气时间短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。研究组住院时间(11.96±1.31)d短于对照组的(19.02±1.62)d,差异有统计学意义(P<0.05),两组住院费用对比差异无统计学意义(P>0.05)。研究组术后并发症发生率3.70%低于对照组的29.63%,差异有统计学意义(P<0.05)。结论肝硬化门静脉高压症患者应用腹腔镜巨脾切除联合断流术能够有效减少手术时间、术中出血量、肛门排气时间,降低并发症发生率,同时还能够显著缩短患者住院时间,效果显著。Objective To study the clinical effect of laparoscopic splenectomy combined with devascularization in the treatment of portal hypertension due to liver cirrhosis.Methods A total of 54 patients with portal hypertension due to liver cirrhosis were divided into a control group and a research group according to different treatment methods,with 27 cases in each group.The control group was treated with traditional laparotomy,and the research group was treated with laparoscopic splenectomy and devascularization.The clinical indicators,hospitalizations,and complication rates of the two groups were compared.Results The operation time and anal exhaust time of the research group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).The hospitalization time was(11.96±1.31)d in the research group,which was shorter than(19.02±1.62)d in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in hospitalization expenses between the two groups(P>0.05).The postoperative complication rate was 3.70%in the research group,which was lower than 29.63%in the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of laparoscopic splenectomy and devascularization has significant effect on patients with portal hypertension due to liver cirrhosis,and it can effectively reduce the operation time,intraoperative blood loss,anal exhaust time and lower the complication rates,but also can significantly shorten the hospitalization time of patients.
