彩超诊断胆囊管残端扩张继发胆总管结石的临床价值

摘　　要：目的彩色多普勒超声(彩超)诊断在胆囊管残端扩张继发胆总管结石中的应用价值。方法72例胆囊管残端扩张继发胆总管结石患者,均实施磁共振胰胆管造影(MRCR)与彩超检查。比较两种方法结石显示率、确诊率、漏诊率及检查费用。结果两种方法结石显示率比较差异无统计学意义(P>0.05)。彩超的确诊率97.22%(70/72)高于MRCR的87.50%(63/72),漏诊率1.39%(1/72)低于MRCR的11.11%(8/72),差异具有统计学意义(χ^(2)=4.823、5.807,P=0.028、0.016<0.05)。彩超的检查费用为(324.15±54.11)元,少于MRCR的(928.12±112.41)元,差异具有统计学意义(t=41.079,P=0.000<0.05)。结论彩超诊断对于胆囊管残端扩张继发胆总管结石患者具有应用价值。Objective To study the practical value of color Doppler ultrasonography in the diagnosis of choledocholithiasis secondary to dilatation of cystic duct remnant.Methods There were 72 patients with choledocholithiasis secondary to dilatation of cystic duct remnant,and magnetic resonance cholangiopancreatography(MRCR)and color Doppler ultrasonography were performed in all the patients.The stone display rate,diagnosis rate,missed diagnosis rate and examination cost were compared between the two methods.Results There was no statistically significant difference in stone display rate between the two methods(P>0.05).The diagnosis rate of color Doppler ultrasonography 98.61%(71/72)was higher than 88.89%(64/72)of MRCR,and the missed diagnosis rate 1.39%(1/72)was lower than 11.11%(8/72)of MRCR.All the difference was statistically significant(χ^(2)=5.807,5.807;P=0.016,0.016<0.05).The examination cost of color Doppler ultrasonography was(324.15±54.11)yuan,which was less than(928.12±112.41)yuan of MRCR,and the difference was statistically significant(t=41.079,P=0.000<0.05).Conclusion Color Doppler ultrasonography is valuable in patients with choledocholithiasis secondary to dilatation of cystic duct remnant.
