等离子针状电极整块切除术与环状电切术治疗非肌层浸润性膀胱癌的临床效果比较

摘　　要：目的比较等离子针状电极整块切除术和环状电切术治疗非肌层浸润性膀胱癌的临床效果。方法选取2017年11月至2020年11月我院及新疆医科大学第一附属医院收治的98例非肌层浸润性膀胱肿瘤患者,按手术方式将其分为等离子针状电极组(A组)与传统环状电切组(B组),每组49例。比较两组的手术效果。结果A组的肿瘤切除耗时短于B组,术中出血量少于B组,闭孔神经反射发生率低于B组,肿瘤病理标本完整度高于B组(P<0.05)。A组的术后住院时间、导尿管留置时间短于B组,术后1年内的复发率、疼痛评分低于B组(P<0.05)。A组的并发症总发生率低于B组(P<0.05)。结论经尿道等离子针状电极整块切除术治疗非肌层浸润性膀胱癌的效果安全有效,可有效降低闭孔神经反射、更利于病理观察。Objective To compare the clinical effect of plasma needle electrode block resection and circular resection in the treatment of non-muscle invasive bladder cancer.Methods A total of 98 patients with non-muscle invasive bladder cancer treated in our hospital and the first affiliated hospital of Xinjiang medical university from November 2017 to November 2020 were selected and divided into plasma needle electrode group(group A)and traditional circular electroresection group(group B),with 49 cases in each group.The surgical effects of the two groups were compared.Results The tumor resection time in the group A was shorter than that in the group B,the amount of intraoperative bleeding was less than that in the group B,the incidence of obturator nerve reflex was lower than that in the group B,and the tumor pathological specimen integrity was higher than that in the group B(P<0.05).The postoperative hospital stay and indwelling time of urinary catheter in the group A were shorter than those in the group B,and the recurrence rate within 1 year and pain score after operation were lower than those in the group B(P<0.05).The total incidence of complications in the group A was lower than that in the group B(P<0.05).Conclusion Transurethral plasma needle electrode block resection is safe and effective in the treatment of non-muscle invasive bladder cancer.It can effectively reduce obturator nerve reflex and facilitate pathological observation.
