中药灌肠结合负压封闭引流术治疗肛周深部脓肿的临床效果

摘　　要：目的分析中药灌肠结合负压封闭引流术治疗肛周深部脓肿的临床效果。方法选取2017年1月至2019年12月本院收治的58例肛周深部脓肿患者作为研究对象,以数字随机法将其分为参照组和研究组,各29例。参照组采用负压封闭引流术,研究组采用中药灌肠结合负压封闭引流术。比较两组的治疗效果。结果研究组的住院时间、创面愈合时间短于参照组,差异具有统计学意义(P<0.05)。治疗后,两组的白血胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平均降低,且研究组低于参照组,差异具有统计学意义(P<0.05)。治疗后,两组的白血胞介素-4(IL-4)、白血胞介素-10(IL-10)、血管内皮生长因子(VEGF)水平均升高,且研究组高于参照组,差异具有统计学意义(P<0.05)。治疗后,两组的VAS、Wexner评分均降低,且研究组低于参照组,差异具有统计学意义(P<0.05)。结论中药灌肠结合负压封闭引流术治疗肛周深部脓肿的效果显著,可缩短患者住院时间,促进创面愈合,减轻炎症反应及疼痛程度,改善肛门功能。Objective To analyze the clinical effect of traditional Chinese medicine enema combined with vacuum sealing drainage in the treatment of deep perianal abscess.Methods A total of 58 patients with deep perianal abscess treated in our hospital from January 2017 to December 2019 were selected as the research objects,and the patients were divided into reference group and study group by digital random method,with 29 cases in each group.The reference group was treated with vacuum sealing drainage,and the study group was treated with traditional Chinese medicine enema combined with vacuum sealing drainage.The therapeutic effects of the two groups were compared.Results The hospital stay and wound healing time in the study group were shorter than those in the reference group,and the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)in the two group decreased,and those in the study group were lower than the reference group,the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-4(IL-4),interleukin-10(IL-10)and vascular endothelial growth factor(VEGF)in the two group increased,and those in the study group were higher than the reference group,the differences were statistically significant(P<0.05).After treatment,the VAS and Wexner scores in the two groups decreased,and those in the study group were lower than the reference group,the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine enema combined with vacuum sealing drainage in the treatment of deep perianal abscess has a significant effect,which can shorten the hospital stay,promote wound healing,reduce inflammatory reaction and degree of pain,and improve anal function.
