摘  要:作为一种慢性炎症性皮肤病,银屑病以红斑丘疹伴鳞屑为主要表现,部分还伴有脓疱。目前其发病机制尚不明确,可能与辅助性T细胞17/白细胞介素-23轴相关。银屑病患者的肠道菌群呈多样性,且肠道菌群代谢产物[特别是短链脂肪酸(SCFAs)]与银屑病的发病机制存在特定关系,SCFAs可通过“肠-脑-皮轴”调节免疫系统和内分泌系统,进而参与某些炎症性疾病的发病。肠道菌群失调与SCFAs的改变均可能参与银屑病的发生发展,而益生菌的使用则可改善银屑病患者的皮疹,提高患者的生活质量。未来,肠道菌群及其代谢产物对免疫调节和炎症反应的调控机制将成为银屑病相关研究的新方向,以丰富临床对银屑病与肠道菌群的认知。As a chronic inflammatory skin disease,psoriasis is mainly manifested by erythema papules with scales,and some are accompanied by pustules.At present,its pathogenesis is not clear,and it may be related to the helper T cell 17/interleukin-23 axis.The intestinal flora of the patients with psoriasis is diverse,and the metabolites of the intestinal flora,especially short-chain fatty acids(SCFAs),have a specific relationship with the pathogenesis of psoriasis.SCFAs can pass through the"gut-brain-skin axis"to regulate the immune system and endocrine system,and then participate in the pathogenesis of certain inflammatory diseases.Intestinal flora imbalance and changes in SCFAs may be involved in the occurrence and development of psoriasis,and the use of probiotics can improve the rash symptom and improve the quality of life of the patients.In the future,the regulation mechanism of immune regulation and inflammatory response by intestinal flora and its metabolites will become a new direction of psoriasis related studies,which will enrich the clinical understanding of psoriasis and intestinal flora.
