中药联合主动免疫疗法治疗原因不明复发性流产疗效分析

摘　　要：目的:系统评价中药联合主动免疫疗法治疗原因不明复发性流产的临床疗效。方法:计算机检索中国知网(CNKI)、万方数据库(WANFANG DATA)、维普数据库(VIP)、中国生物医学文献数据库(CBM),Cochrane Library、PUBMED等中外数据库,查找中药联合主动免疫疗法治疗原因不明复发性流产的随机对照试验,检索时间限为各数据库建库至2020年7月20日。由两位研究员独立进行文献检索、筛选、质量评价和数据提取,采用RevMan5.3软件进行Meta分析。结果:共纳入10项研究,总样本量为867例。Meta分析结果显示:治疗组妊娠至12周胎儿存活率[RR=1.20,95%CI(1.11,1.31),P<0.0001]、活产率[RR=1.30,95%CI(1.16,1.47),P<0.00001]、T淋巴细胞亚群CD4+水平[MD=-3.19,95%CI(-6.29,-0.10),P=0.04]、T淋巴细胞亚群CD8+水平[MD=1.82,95%CI(0.72,2.93),P=0.001]、CD4+/CD8+水平[MD=-0.14,95%CI(-0.24,-0.04),P=0.007]均优于对照组。结论:中药联合主动免疫疗法可提高原因不明复发性流产患者妊娠至12周胎儿存活率、活产率、T淋巴细胞亚群CD8+水平,降低T淋巴细胞亚群CD4+水平、CD4+/CD8+水平,且优于单独使用主动免疫疗法。Objective:To systematically evaluate the clinical efficacy of Chinese medicine combined with active immunotherapy in the treatment of unexplained recurrent miscarriage.Methods:Computer search of China Knowledge Network(CNKI),Wanfang database(WANFANG DATA),VIP database(VIP),Chinese Biomedical Literature Database(CBM),Cochrane Library,PUBMED and other Chinese and foreign databases to find out the reasons for the treatment of Chinese medicine combined with active immunotherapy For randomized controlled trials of unidentified recurrent miscarriage,the search time is limited to the establishment of each database until July 20,2020.Two researchers independently carried out literature search,screening,quality evaluation and data extraction,and used RevMan5.3 software for meta-analysis.Results:A total of 10 studies were included,with a total sample size of 867 cases.The results of Meta analysis showed that the survival rate of the fetus in the treatment group from pregnancy to 12 weeks[RR=1.20,95%CI(1.11,1.31),P<0.0001],live birth rate[RR=1.30,95%CI(1.16,1.47),P<0.00001],T lymphocyte subgroup CD4+level[MD=-3.19,95%CI(-6.29,-0.10),P=0.04],T lymphocyte subgroup CD8+level[MD=1.82,95%CI(0.72,2.93),P=0.001],CD4+/CD8+levels[MD=-0.14,95%CI(-0.24,-0.04),P=0.007]are better than the control group.Conclusion:Chinese medicine combined with active immunotherapy can improve the fetal survival rate,live birth rate,and CD8+levels of T lymphocyte subsets in patients with unexplained recurrent miscarriage from pregnancy to 12 weeks,and reduce the CD4+levels and CD4+/CD8+levels of T lymphocyte subsets,and It is better than active immunotherapy alone.
