正骨手法复位小夹板固定治疗成人前臂双骨折的临床疗效

摘　　要：目的研究成人前臂双骨折治疗中正骨手法复位小夹板固定的临床疗效。方法采用回顾性分析方法,以2018年6月至2020年6月四川省骨科医院收治入院的76例成人前臂双骨折患者为研究对象,根据手术方式的不同分为对照组(38例)与研究组(38例);对照组行切开复位钢板内固定术治疗,研究组行正骨手法复位小夹板固定术治疗。比较2组患者手术前后疼痛评分、生活质量评分与术后住院时间、骨折愈合时间、骨折康复优良率。结果术后,2组患者疼痛评分较术前明显降低,生活质量评分较术前明显提高,差异均有统计学意义(P<0.05);研究组术后疼痛评分为(1.58±0.81)分,低于对照组[(2.24±0.73)分],生活质量评分与骨折康复优良率(92.11%)高于对照组(71.05%),骨折愈合时间、住院时间为(5.76±1.73)d、(6.14±1.11)周,短于对照组[(2.24±0.73)分、(8.87±1.68)周],差异均有统计学意义(P<0.05)。结论成人前臂双骨折治疗中正骨手法复位小夹板固定的临床疗效明显优于切开复位钢板内固定,可减轻患者的疼痛程度,提高生活质量,促使其骨折快速愈合。Objective To study the clinical effect of small splint fixation in the treatment of adult forearm double fracture by bone setting manipulation.Methods The retrospective analysis methods was used and 76 adult patients with double forearm fractures admitted Sichuan Orthopaedic Hospital from June 2018 to June 2020 were selected as the research objects.According to the operation method,they were divided into the control group(38 cases)and the study group(38 cases);control group was treated with open reduction and internal fixation with steel plate,and the study group was treated with manual reduction and splint fixation.The pain score,quality of life score,postoperative hospital stay,fracture healing time,and fracture recovery rate were compared between the two groups of patients before and after surgery.Results After operation,the pain score in the two groups of patients were significantly lower than those before the operation,and the quality of life scores were significantly higher than those before the operation,the differences were statistically significant(P<0.05).The postoperative pain score in the study group were(1.58±0.81)points,which were lower than those in the control group[(2.24±0.73)points],the quality of life scores and fracture rehabilitation rate(92.11%)were higher than those in the control group(71.05%),the fracture healing time and hospital stay were(5.76±1.73)days and(6.14±1.11)weeks,which were shorter than those in the control group[(2.24±0.73)minutes,(8.87±1.68)weeks],and the differences were statistically significant(P<0.05).Conclusion In the treatment of adult double fractures of the forearm,the clinical effect of manual reduction and splint fixation is significantly better than that of open reduction and steel plate internal fixation,which can reduce the patient's pain,improve the quality of life,and promote the rapid healing of their fractures.
