摘  要:目的应用静息态功能磁共振成像(rs-fMRI)探讨单侧肢体无力孤立急性桥脑梗死患者静息态低频振幅变化情况。方法采用前瞻性研究方法,搜集2018年7月至2020年1月于首都医科大学附属北京友谊医院急诊初次就诊、31例表现为单侧肢体无力孤立急性桥脑梗死患者,并招募31例年龄、性别及教育程度相匹配的健康志愿者行rs-fMRI。每组分别进行低频振幅(ALFF)分析,运用双样本t检验比较组间在全脑水平上大脑活动的变化,并对差异脑区的ALFF值与美国国立卫生研究院卒中量表(NIHSS)评分进行相关性分析。结果急性桥脑梗死后远隔部位脑区功能活动发生改变(Bonferroni cluster水平校正,P<0.05),两组相比ALFF值升高的脑区有右侧岛叶、双侧顶下缘角回、双侧颞下回、右侧背外侧额上回;ALFF值降低的脑区包括左侧内侧旁扣带回、双侧丘脑、右侧小脑半球。病例组中左侧顶下缘角回(r=0.422,P=0.018)、左侧颞下回(r=0.355,P=0.050)的ALFF值与NIHSS评分呈显著正相关。结论急性桥脑梗死患者远隔脑区自发脑神经功能活动会发生改变,可能由于相关脑区的“低能”“低效”表现,临床神经功能缺损越严重的患者,优势半球远隔脑区自发神经活动变化越大。Objective To investigate amplitude of low-frequency fluctuation(ALFF)of brains in patients with limb weakness acute pontine infarction using resting-state functional MRI(rs-fMRI).Methods Prospective research methods were used.There were thirty-one participants with unilateral limb weakness acute isolated pontine infarction from the Department of Emergency,Beijing Friendship Hospital affiliated to Capital Medical University took part in the study from July 2018 to January 2020.Thirty-one healthy volunteers with age,gender and education matched to the study group also participated as the control group.These two groups underwent rs-fMRI and the results were then converted to the low-frequency fluctuation data.These ALFF data were further analyzed by two-sample t test among two groups based on the whole brain levels.ALFF results were subjected to t-test of voxel wised comparison to derive the rs-fMRI brain function differences between the two groups.The correlation between ALFF values of abnormal regions found in two groups and the scores obtained according to the National Institute of Health stroke scale(NIHSS)were analyzed.Results The two groups had shown significant differences in terms of the ALFF results,with the ALFF increased in several brain regions(Bonferroni cluster,P<0.05),which include right insula gyrus,bilateral inferior parietal marginal angular gyrus,bilateral inferior temporal gyrus,right dorsolateral superior frontal gyrus.The two groups had shown a significant differences(ALFF decreased)in several brain regions(Bonferroni cluster,P<0.05),including left medial lateral cingulate gyrus,bilateral thalamus and right cerebellum.In addition,the ALFF values of left inferior parietal marginal angular gyrus and left inferior temporal gyrus had a positive correlation with NIHSS scores(r=0.422,P=0.018;r=0.355,P=0.050 respectively).Conclusion Acute isolated pontine infarction group with unilateral limb weakness had abnormal brain activity in several remote brain regions served to motor network,these regions can
