摘　　要：目的探讨实时三维超声心动图(RT-3DE)评价心肌梗死患者左心室收缩功能的应用价值,为临床诊断提供参考。方法回顾性选取于2018年1月至2019年12月来鞍山市中心医院接受治疗的已确诊左心室心肌梗死患者96例,根据梗死部位分为下壁组(n=22)、后壁组(n=26)、前壁及前间壁组(n=24)、侧壁组(n=24)。先后利用RT-3DE全容积法、心脏磁共振成像(CMRI)测量并比较4组患者左心室舒张末容积(LVEDV)、每搏输出量(SV)、左心房内径(LAD)、左心室射血分数(LVEF)参数值及相关性。结果RT-3DE全容积法与CMRI测得4组的LVEDV、SV、LAD、LVEF间差异均无统计学意义(P>0.05);RT-3DE全容积法与CMRI测量得出LVEDV、SV、LAD、LVEF之间有良好的相关性(下壁组r=0.91、0.86、0.90、0.88,后壁组r=0.90、0.89、0.89、0.90,前壁及前间壁组r=0.89、0.91、0.92、0.91,侧壁组r=0.89、0.88、0.91、0.89,P<0.05)。结论RT-3DE技术能够准确反映心肌梗死患者的左心室收缩功能,与CMRI检测结果具有相关性,是一种能够为临床全面评价左心功能提供较可靠信息的新方法,值得基层推广应用。Objective To investigate the application value of real-time three-dimensional echocardiography(RT-3DE)in evaluating left ventricular systolic function in patients with myocardial infarction,so as to provide reference for clinical diagnosis.Methods Ninety-six patients with confirmed left ventricular myocardial infarction who came to Anshan Central Hospital for treatment from January 2018 to December 2019 were retrospectively selected and divided into inferior wall group(n=22)and posterior wall group(n=26),anterior wall and anterior partition group(n=24),lateral wall group(n=24)according to the location of the infarction.The levels and correlations of left ventricular end diastolic volume(LVEDV),stroke volume(SV),left atrial diameter(LAD),and left ventricular ejection fraction(LVEF)in the 4 groups were successively used by RT-3DE full volume method,cardiac magnetic resonance imaging(CMRI)was measured and compared.Results There was no statistically significant difference between the three-dimensional echocardiographic full-volume method and CMRI in the four groups of LVEDV,SV,LAD,and LVEF(P>0.05).There was a good correlation between,LAD and LVEF measured by the three-dimensional echocardiographic full-volume method and CMRI measured LVEDV and SV(inferior wall group r=0.91,0.86,0.90,0.88,posterior wall group r=0.90,0.89,0.89,0.90,anterior wall group r=0.89,0.91,0.92,0.91,side wall group r=0.89,0.88,0.91,0.89,P<0.05).Conclusion In conclusion,RT-3DE technology can accurately reflect the left ventricular systolic function in patients with myocardial infarction.It was correlated with the results of CMRI.It is a new method that can provide more reliable information for clinical comprehensive evaluation of left ventricular function,and is worthy of popularization and application in grass-roots units.
