摘  要:目的研究p-Akt、p-4EBP1、Ki-67和TopoIIa蛋白在正常子宫内膜、无不典型性的子宫内膜增生及子宫内膜不典型增生中的表达情况及相关性。方法采用回顾性研究,收集2014年1月至2018年6月首都医科大学附属北京友谊医院病理科已确诊存档蜡块。通过免疫组化的方法检测30例EM组、30例EH组和20例AEH组子宫内膜中p-Akt、p-4EBP1、Ki-67和TopoIIa表达差异以及免疫反应分布和强度积分法(IRS)相关性情况。结果P-Akt在EM组、EH组及AEH组中表达阳性率为10.00%、40.00%和50.00%。P-4EBP1在EM组、EH组及AEH组中表达阳性率为10.00%、33.33%和40.00%。Ki-67在EM组、EH组及AEH组中表达阳性率为10.00%、33.33%和40.00%。TopoIIa在EM组、EH组及AEH组中表达阳性率为13.33%、36.67%和55.00%。EH组和AEH组中p-Akt、p-4EBP1、Ki-67和TopoIIa表达阳性率显著高于EM组,差异均有统计学意义(P<0.05),各指标在AEH组表达阳性率显著高于EH组,差异均有统计学意义(P<0.05)。P-Akt、p-4EBP1的IRS与Ki-67的IRS呈正相关(r=0.422,P<0.05;r=0.425,P<0.05),p-Akt、p-4EBP1和TopoIIa的IRS呈正相关(r=0.597,P<0.05;r=0.673,P<0.05)。结论P-Akt、p-4EBP1、Ki-67和TopoIIa在EH及AEH组中高表达,在EM组中低表达。P-Akt与Ki-67、TopoIIa和、p-4EBP1与Ki-67、TopoIIa在EH及AEH组中的正相关关系提示了它们在子宫内膜增生中发挥的重要作用。Objective To study the expression of p-Akt,p-4EBP1,Ki-67 and TopoIIa proteins and their relevance in normal endometrium,endometrial hyperplasia without atypia and endometrial atypia.Methods A retrospective study was used to collect confirmed archived wax blocks from January 2014 to June 2018 from the Department of Pathology,Beijing Friendship Hospital,Capital Medical University,and to detect differences in p-Akt,p-4EBP1,Ki-67 and TopoIIa expression in the endometrium of 30 EM,30 EH and 20 AEH groups and immunoreactive score(IRS)correlation by immunohistochemistry.Results The positive expression rates of p-Akt in the EM,EH,and AEH groups were 10.00%,40.00%,and 50.00%.P-4EBP1 in the EM,EH,and AEH groups were 10.00%,33.33%,and 40.00%.Ki-67 in the EM,EH,and AEH groups were 10.00%,33.33%,and 40.00%.The positive rates of TopoIIa expression in EM,EH and AEH groups were 13.33%,36.67%and 55.00%.p-Akt,p-4EBP1,Ki-67 and TopoIIa expression in Eh and AEH groups were significantly higher than those in EM group,the differences were statistically significant(P<0.05).The positive expression rate of each index was significantly higher in the AEH group than in the AEH group,the differences were statistically significant(P<0.05).P-Akt,p-4EBP1 IRS was positively correlated with the IRS of Ki-67(r=0.422,P<0.05;r=0.425,P<0.05),p-Akt,p-4EBP1,p-4EBP1 and Ki-67(r=0.422,P<0.05).The IRS of Akt,p-4EBP1 and TopoIIa were positively correlated(r=0.597,P<0.05;r=0.673,P<0.05).Conclusion P-Akt,p-4EBP1,Ki-67,and TopoIIa are highly expressed in with and without atypical endometrial hyperplasia and low in normal endometrium.The positive correlation of p-Akt and p-4EBP1 with Ki-67 and TopoIIa implies that they play an important role in endometrial hyperplasia.
