摘　　要：目的探讨乳腺淋巴瘤多模态超声表现,以提高此罕见病诊断的准确率。方法回顾性分析经病理确诊为乳腺淋巴瘤的24例患者的超声表现,包括灰阶超声、彩色多普勒、弹性成像及超声造影。结果乳腺淋巴瘤主要超声表现为形态不规则、边缘模糊的片状高回声肿块,后方回声增强或无明显改变。血流Adler分级以Ⅱ~Ⅲ级居多,应变弹性2~3分占绝大多数,入组病例均未见钙化。造声造影多表现为弥漫高增强,向心性灌注,边界较清,多数未见明显缺损区。病理类型以弥漫大B性淋巴瘤多见。结论乳腺淋巴瘤的超声特点具有其特征性,结合多种超声检查方法有助于该病的诊断。Objective To investigate the multimodal ultrasound features of breast lymphoma and improve the accuracy of preoperative diagnosis of this rare disease. Methods The characteristics of breast lymphoma in ultrasound, including gray-scale ultrasound, color Doppler, elastography and contrast-enhanced ultrasound, were analyzed retrospectively in 24 patients diagnosed by pathology. Results The mainly ultrasound features of breast lymphomas were manifested as hyperechoic spot with irregular shapes and fuzzy edges, with posterior acoustic enhancement or not. The Adler classification of blood flow was mostly from grade Ⅱ to Ⅲ, and the strain elasticity score was 2-3 points accounting for the vast majority. None of calcification was found in the enrolled cases. The characteristic of contrast-enhanced ultrasound was mostly manifested as diffuse hyper-enhancement, centripetal perfusion, clear borders, and most of them didn′t present obviously perfusion defects. The pathological type was more common with diffuse large B-cell the disease. Conclusions Breast lymphoma have unique ultrasound characteristics, multimodal ultrasound could improve the differential diagnosis of lymphoma.
