双重超声造影联合超声内镜在胃癌患者术前TNM分期的应用价值

摘　　要：目的探讨双重超声造影(DCEUS)联合超声内镜(EUS)在胃癌患者术前TNM分期的应用价值。方法回顾性分析89例病理确诊的胃癌患者术前DCEUS联合EUS及增强CT检查的影像表现,并行TNM分期,比较DCEUS联合EUS检查TNM分期与增强CT分期的一致性。结果 DCEUS联合EUS检查、增强CT检查对胃癌患者术前T分期、N分期的总体准确率比较差异无统计学意义(P>0.05),两者T1分期准确率比较差异有统计学意义(P<0.05);两者诊断M+分期的Kappa值为0.700,一致性程度较好。结论 DCEUS联合EUS检查在胃癌患者术前TNM分期上有较高的诊断价值,可为临床胃癌的术前分期诊断提供有效信息。Objective To explore the value of double contrast-enhanced ultrasonography(DCEUS) combined with endoscopic ultrasonography(EUS) in preoperative TNM staging of patients with gastric cancer. Methods 89 cases of patients with gastric cancer were retrospectively analyzed. TNM staging were evaluated by DCEUS, EUS and enhanced CT. The accuracy of TNM staging for DCEUS combined with EUS and enhanced CT were compared. Results The overall accuracy of DCEUS combined with EUS and enhanced CT in T staging, N staging were no significant difference(P>0.05). Particularly, The difference of T1 staging accuracy between the two groups was statistically significant(P<0.05). The Kappa value of M+ staging was 0.700, suggesting a good consistency between the two groups. Conclusions DCEUS combined with EUS has high accuracy in preoperative TNM staging of patients with gastric cancer. It might be useful for diagnosing the TNM staging of gastric cancers before surgery.
