摘　　要：目的对超声监测水压灌肠整复小儿原发性肠套叠疗效的影响因素进行分析和预测。方法回顾性分析187例确诊原发性肠套叠并采取超声下水压灌肠整复患儿的临床资料,分为成功组与失败组。单因素采用χ2检验及t检验方法分析,筛选出与整复结果相关度较高的多项因素,进行二元Logistic回归分析得出影响整复结果的独立影响因素,绘制ROC曲线,评价其预测能力。结果患儿病程、超声征象"同心圆"直径和"套筒"长度是影响整复结果的独立影响因素;三者ROC曲线下面积分别为0.758、0.785、0.871,三者截断值分别为21.75 h、27.5 mm、52.5 mm。结论肠套叠患儿病程、超声"同心圆"直径和"套筒"长度对超声监测水压灌肠整复结果有较好的预测能力,为临床选择复位治疗方式提供指导依据。Objective To analyze and predict the influencing factors of the effect of the hydraulic enema on primary intussusception in children. Methods The clinical data of 187 children diagnosed with primary intussusception were retrospectively analyzed, and they were divided into successful groups and failed groups. Single-factor χ2 test and T-test were used to screen out multiple factors highly correlated with the results of the rectification. Binary Logistic regression analysis was carried out to find the independent factors affecting the results of the rectification, and ROC curve was drawn to evaluate its predictive value. Results The course of the disease, the diameter of the "concentric circle" and the length of the "sleeve" of ultrasonic signs were independent factors that affected the results of the reconstruction. The areas under the ROC curve of the three were 0.758, 0.785, and 0.871, respectively, and the cutoff values of the three were 21.75 h, 27.5 mm, and 52.5 mm, respectively. Conclusions In children with intussusception, the course of the disease, the diameter of the "concentric circle" and the length of the "sleeve" have a good predictive value for the results of the ultrasonic hydraulic enema reduction, which provides guidance for the clinical selection of reduction treatment.
