涎腺淋巴上皮病变超声图像分析及鉴别诊断

摘　　要：目的探讨涎腺淋巴上皮病变超声图像特征,提高超声对其诊断水平。方法回顾性分析10例经病理证实为涎腺淋巴上皮病变超声图像特征,包括部位、大小、形态、边界、内部回声、血流信号等特征。结果 10例患者共计15个病灶,良性病变可表现为形态规则、边界尚清的蜂窝样混合回声或囊肿样无回声,部分病灶内可见多发点状强回声后方伴或不伴彗星尾征,血流信号较为丰富;恶性病变主要表现为形态不规则、边界模糊的实性低回声,血流较少。结论涎腺淋巴上皮病变超声图像具有特征性,掌握这些特征可以提高超声对其诊断水平。Objective To analyze the ultrasonographic features of salivary gland lymphoepithelial lesions and improve the diagnostic level of ultrasonography. Methods The ultrasonographic features of 10 patients with pathologically confirmed lymphoepithelial lesions of salivary gland were analyzed retrospectively, including location, size, shape, boundary, internal echo and blood flow signal. Results There were 15 lesions in 10 patients. The benign lesions showed honeycomb like mixed echo or cyst like non echo with regular shape and clear boundary. Multiple or single punctate hyperechoic lesions were found in some lesions, with or without comet tail sign, and the blood flow signals were abundant;Malignant lesions were mainly characterized by irregular shape, fuzzy boundary of solid hypoechoic, blood flow for the lack of blood supply type. Conclusions The ultrasound images of salivary gland lymphoepithelial lesions have characteristics, and mastering these characteristics can improve the diagnostic level of ultrasound.
