摘　　要：目的通过分析不同麻醉深度下拨管对麻醉恢复室(PACU)转运的影响,以选择恰当的拔管时机,提高患者安全系数及患者周转速度。方法全麻气管插管手术患者60例,分为Ⅰ组(脑电双频指数,BIS 56~65)、Ⅱ组(BIS 66~75)和Ⅲ组(BIS≥76),每组20例。记录各组患者入PACU时(T0),拔管时(T1),拔管后1min(T2)、5min(T3)、10min(T4)的平均动脉压(MAP)和心率(HR);观察不同麻醉深度下拔管的并发症;记录患者清醒时间和留观时间。结果Ⅰ组和Ⅱ组患者各时间段内MAP,HR平稳,差异无统计学意义(P>0.05),T1,T2时间段Ⅲ组的MAP,HR显著上升(P<0.05);Ⅱ组不良反应发生率低于Ⅰ组和Ⅲ组;与Ⅰ组相比,Ⅱ组和Ⅲ组的清醒时间和留观时间短,差异具有统计学意义(P<0.05)。结论BIS值在66~75区间内拔管不良反应发生少,清醒时间与留观时间短,加快PACU转运。Objective To analyze the effect of different depth of anesthesia on the efficiency of Postanesthesia care unit,so as to choose the appropriate time of extubation,improve the safety of the patient and the speed of patient turnover.Methods Sixty patients with tracheal intubation under general anesthesia were divided intoⅠgroup(BIS 56~65),Ⅱgroup(BIS 66~75)andⅢgroup(BIS≥76),with 20 cases in each group.Mean arterial pressure and heart rate at admission to PACU(T0),extubation(T1),1min(T2),5min(T3)and 10min(T4)were recorded in each group.The complications of extubation under different anesthesia depth were observed.Waking time and observing time were recorded.Results The MAP and HR of theⅠgroup and theⅡgroup were stable in each time period,and the difference was not statistically significant(P>0.05).The MAP and HR of theⅢgroup were significantly increased in T1 and T2 time periods(P<0.05).The incidence of adverse reaction inⅡgroup was lower than that inⅠgroup andⅢgroup.Compared with theⅠgroup,the waking time and observing time were shorter in theⅡgroup andⅢgroup,and the difference was statistically significant(P<0.05).Conclusion The extubation in 66~75 BIS has less adverse reactions and shorter waking time and observation time,which can accelerate the efficiency of PACU.
