摘  要:目的探讨小儿供氧-吸痰双腔管在先天性心脏病(CHD)心脏手术后患儿低氧血症吸痰中的应用。方法2019年2月~2020年12月在体外循环下行心脏手术后入住监护室应用机械辅助通气且发生低氧血症的患儿20例,采用自身对照的方法按需吸痰,未吸痰组为正常机械辅助通气患儿,单腔组为使用普通单腔吸痰管吸痰,双腔组为使用供氧-吸痰双腔管吸痰,分别在吸痰15s时从留置的桡动脉测压管内抽血做血气分析,观察并记录患儿心率(HR)、动脉血氧分压(PaO_(2))、动脉血压(ABP)、末梢血氧饱和度(SpO_(2))。结果单腔组与未吸痰组相比,HR,PaO_(2),ABP,SpO_(2)差异有统计学意义(P<0.01);双腔组与未吸痰组相比,HR,PaO_(2),ABP,SpO_(2)差异有统计学意义(P<0.01)。结论使用小儿供氧-吸痰双腔管在术后低氧血症患儿中对HR,PaO_(2),ABP,SpO_(2)的影响更小,更利于维持患儿血流动力学的稳定。Objective To investigate the clinical observation of pediatric oxygenation-aspiration dual-lumen tube application in children with hypoxemia after cardiac surgery for congenital heart disease(CHD).Methods From February 2019 to December 2020,20 children admitted to the care unit after cardiac surgery under extracorporeal circulation in our cardiac surgery department to apply mechanically assisted ventilation,and developed hypoxemia were aspirated on demand using the self-control method.The unaspirated group were children with normal mechanical assisted ventilation,the single-lumen group were sputum aspirated with a normal single-lumen aspiration tube,and the dual-lumen group were sputum aspirated with a dual-lumen oxygenation-aspiration tube.Blood was drawn from the retained radial artery manometry tube for blood gas analysis at 15s of sputum aspiration,respectively.Heart rate(HR),the arterial partial pressure of oxygen(PaO_(2)),arterial blood pressure(ABP),and peripheral oxygen saturation(SpO_(2))were observed and recorded.Results The differences in HR,PaO_(2),ABP,and SpO_(2) were statistically significant in the single-chamber group compared with the unaspirated group(P<0.01).There were statistically significant differences in HR,PaO_(2),ABP,and SpO_(2) in the double-chambered group compared with the unaspirated group(P<0.01).Conclusion The use of the pediatric oxygenation-aspiration double-lumen tube developed by the author has less effect on HR,PaO_(2),ABP,and SpO_(2) in children with postoperative hypoxemia and is more conducive to maintaining hemodynamic stability in children.
