血清hs-CRP、HCY在壮族骨质疏松性骨折患者中的水平及临床意义

摘　　要：目的探讨壮族骨质疏松(OP)性骨折患者血清超敏C反应蛋白(hs-CRP)、同型半胱氨酸(HCY)水平及临床意义。方法选择骨折患者117例,其中骨质疏松组60例,非骨质疏松组57例,采用免疫比浊法测定hs-CRP水平,采用酶循环法测定HCY水平。结果OP组中血清hs-CRP、HCY水平较非OP组的水平高(P<0.05),调整年龄、性别及体质指数后,以hs-CRP、HCY作为自变量的多元Logistic回归显示,hs-CRP水平升高与OP骨折相关性有统计学意义(OR=1.189,95%CI:1.082~1.307,P<0.001),并独立于OP骨折的传统危险因素。HCY与OP骨折的相关性无统计学意义(OR=1.204,95%CI:0.951~1.523,P=0.123)。结论壮族中老年骨质疏松患者血清中hs-CRP、HCY水平升高,其中hs-CRP与壮族OP骨折有一定关联,hs-CRP增高是OP骨折的危险因素。hs-CRP可能是今后壮族人群骨质疏松症防治潜在的生物标志物。Objective To investigate the levels of serum hypersensitive C-reactive protein(hs-CRP)and homocysteine(HCY)and their clinical significance in Zhuang ethnic patients with osteoporotic(OP)fracture.Methods 117 patients with fracture were selected,including 60 patients in the OP group and 57 patients in the non-OP group.The level of hs-CRP was measured by Turbidimetric Inhibition Immunoassay,and the level of HCY was measured by enzymatic cycling assay.Results The serum levels of hs-CRP and HCY in the OP group were higher than those in the non-OP group(P<0.05).After adjusting for age,gender and body mass index,multiple Logistic regression with hs-CRP and HCY as independent variables showed that the increase of hs-CRP level was significantly correlated with OP fracture(OR=1.189,95%CI:1.082~1.307,P<0.001)and it was independent of traditional risk factors of OP fracture.There was no significant correlation between HCY and OP fracture(OR=1.204,95%CI:0.951~1.523,P=0.123).Conclusion The serum levels of hs-CRP and HCY increase in middle-aged and elderly Zhuang patients with osteoporosis.And hs-CRP is associated with OP fracture in patients of Zhuang ethnic group.The increase of hs-CRP is a risk factor for OP fracture.hs-CRP may be a potential biomarker for the prevention and treatment of OP in Zhuang people in the future.
