稳心颗粒对急性期心房心室主动螺旋电极起搏阈值和心电信号振幅的影响

摘　　要：目的观察稳心颗粒对双腔主动螺旋电极起搏器植入术后合并早搏(房性早搏、室性早搏)患者的起搏阈值、心电信号振幅的影响,以及稳心颗粒对早搏治疗的有效性及安全性。方法新植入双腔起搏器合并早搏患者103例,随机分为观察组(n=50)和对照组(n=53)。观察组:起搏器术后3 d内开始,在口服β受体阻滞剂琥珀酸美托洛尔片的基础上,加用稳心颗粒口服,疗程为90 d;对照组:仅口服琥珀酸美托洛尔片抗心律失常治疗。观察指标:(1)用药前、术后10 d、术后30 d、术后90 d的心房、心室起搏阈值和心房、心室心内电信号振幅。(2)稳心颗粒用药前和用药10 d、30 d、90 d的有效率。(3)安全性评价:检测患者的肝肾功能水平,并观察不良反应(主要包括头晕、恶心呕吐、食欲不振、潮热)发生情况。结果(1)用药前及术后10 d、30 d、90 d,两组患者心房、心室起搏阈值均较为稳定,组内前后未见明显变化;且同期比较,两组间差异无统计学意义(P>0.05);心房、心室心内电信号振幅方面,术后10 d、30 d、90 d,心房P波振幅两组间差异无统计学意义(P>0.05),同期比较,心室R波振幅均稳定在较低水平,两组差异无统计学意义(P>0.05);(2)药物干预10 d、30 d,观察组早搏治疗有效率较对照组更高,差异有统计学意义(P<0.05);治疗90 d,两组间有效率相当,差异无统计学意义(P>0.05)。(3)用药30 d、90 d,两组患者肝、肾功能水平均在正常范围。结论口服稳心颗粒对急性期起搏阈值及心内电信号振幅无明显影响;稳心颗粒联合β受体阻滞剂治疗起搏器术后早搏10 d、30 d有一定疗效;稳心颗粒应用于临床安全性良好。Objective To observe the effect of Wenxin Granule on pacing threshold and ECG amplitude in the patients with premature beats(atrial premature beats and ventricular premature beats)after double chamber active spiral elec⁃trode pacemaker implantation,and the effectiveness and safety of Wenxin Granule in the treatment of premature beats.Methods A total of 103 patients with newly implanted dual chamber pacemaker combined with dual chamber and pre⁃mature beat were randomly assigned into observation group and control group,50 cases for each.The observation group:after 3 d of the pacemaker surgery,based on takingβreceptor blocker Metoprolol Succinate Tablets,Wenxin granule was added orally for 90 days;the control group:treated with Metoprolol Succinate Tablets only.Changes of atri⁃al and ventricular pacing threshold and atrial and ventricular intracardiac electrical signal amplitude before medica⁃tion,10 days,30 days and 90 days after operation;the effective rates of Wenxin Granule before and after treatment for 10,30 and 90 days;Safety evaluation:the level of liver and kidney function and the incidence of adverse reactions(mainly including dizziness,nausea and vomiting,loss of appetite and hot flashes)were detected.Results①The atrial and ventricular pacing thresholds of the two groups were relatively stable before the medication and on the 10,30 and 90 days after operation,and there were no significant changes in the two groups;Compared with the same period,there were no differences between the two groups(P>0.05);in terms of the amplitude of atrial and ventricular intracardiac electrical signals,there were no differences between the two groups at 10,30 and 90 days after operation(P>0.05).Compared with the same period,the amplitude of ventricular R wave was stable at a low level,and there was no differ⁃ence between the two groups(P>0.05);②After drug intervention for 10 days and 30 days,the effective rate of prema⁃ture beats in the observation group was higher than that in the control group(P<0.05);after
