摘 要:目的:探讨膀胱膨出患者膀胱脱垂分型与盆底实时三维超声参数的关联性。方法:回顾性分析某院2019年1月至2020年6月收治的80例膀胱膨出患者以及同期80例健康体检者(女性)的临床资料,分为观察组(膀胱膨出患者)和对照组(健康体检者)。2组受试者均接受盆底实时三维超声检查,比较2组受试者间以及观察组内不同膀胱脱垂分型患者间的尿道旋转角度、膀胱尿道后角变化、膀胱颈移动度等盆底实时三维超声参数,并采用Spearman相关性分析法检验膀胱膨出患者膀胱脱垂分型与盆底实时三维超声参数的相关性。数据采用SPSS 25.0统计学软件进行处理。结果:观察组的尿道旋转角度、膀胱尿道后角变化和膀胱颈移动度均大于对照组(P<0.05),且随着膀胱脱垂分型的升高,3种指标均增大,差异具有统计学意义(P<0.05)。Spearman相关性分析法验证膀胱膨出患者膀胱脱垂分型与盆底实时三维超声参数(尿道旋转角度、膀胱尿道后角变化、膀胱颈移动度)呈正相关性(r=0.52、0.50、0.55,P=0.011、0.015、0.007)。结论:膀胱膨出患者膀胱脱垂分型与盆底实时三维超声参数密切相关,可为临床早期干预和诊治膀胱膨出提供可靠的依据。Objective To study the correlation between the classification of bladder prolapse and the real-time three-dimensional ultrasound parameters of the pelvic floor in patients with cystocele.Methods The clinical data of 80 cystocele patients admitted to some hospital from January 2019 to June 2020 and 80 healthy physical examinees(female)during the same period were retrospectively analyzed and divided into an observation group(patients with cystocele)and a control group(healthy examinees).The subjects in both groups underwent real-time three-dimensional ultrasound examination of the pelvic floor.The pelvic floor real-time three-dimensional ultrasound parameters such as urethral rotation angle,change in posterior vesicourethral angle and bladder neck mobility were compared between the subjects in the groups and between the patients with different bladder prolapse types within the observation group.The correlation between bladder prolapse classification and pelvic floor real-time three-dimensional ultrasound parameters in the cystocele patients was explored using Spearman correlation analysis.The data were processed using SPSS 25.0 statistical software.Results The urethral rotation angle,change in posterior bladder urethral angle and bladder neck mobility were all greater in the observation group than in the control group(P<0.05),and all the three indexes significantly rose with the aggravation of the bladder prolapse(P<0.05).Spearman correlation analysis showed a positive correlation existed between the bladder prolapse classification and real-time three-dimensional ultrasound parameters of the pelvic floor in cystocele patients(r=0.52,0.50,0.55,P=0.011,0.015,0.007).Conclusion The classification of bladder prolapse in cystocele patients is closely related to the rotation angle of the urethra,change in posterior vesicourethral angle and bladder neck mobility,and references are provided for diagnosis and early intervention of bladder prolapse.
