摘　　要：目的观察穴位电刺激联合椎旁神经阻滞治疗急性期带状疱疹神经痛的临床疗效。方法选择急性期带状疱疹(Herpes Zoster,HZ)患者60例,随机分为治疗组和对照组,每组30例。治疗组给予疼痛区域相应节段的夹脊穴和阿是穴电刺激,联合椎旁神经阻滞治疗。对照组给予椎旁神经阻滞治疗。观察两组患者治疗前(T_(0))、治疗后第1天(T_(1))、第7天(T_(2))、第14天(T_(3))的视觉模拟评分(Visual Analogue Scale,VAS)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)评分变化,评价两组治疗方法的临床疗效。结果与T_(0)时比较,T_(1)、T_(2)和T_(3)组内VAS评分降低,并呈现下降趋势,差异有统计学意义(P<0.05);组间比较,T_(0)和T_(1)时的VAS评分差距没有统计意义(P>0.05),T_(2)和T_(3)时治疗组VAS评分下降幅度大于对照组(P<0.05),随着穴位电刺激治疗时间的延长,治疗组在缓解患者疼痛上优于对照组。与T_(0)比较,T_(1)、T_(2)和T_(3)组内HAMA评分均下降,并呈现下降趋势,差异有统计学意义(P<0.05);组间比较,T_(0)和T_(1)时HAMA评分组间差别没有统计意义(P>0.05),T_(2)和T_(3)时治疗组HAMA评分下降幅度大于对照组(P<0.05),随着穴位电刺激治疗时间的延长,治疗组在改善患者焦虑状态方面优于对照组。结论穴位电刺激联合椎旁神经阻滞治疗急性期带状疱疹神经痛疗效确切,且在缓解疼痛和改善患者焦虑状态方面优于单纯椎旁神经阻滞治疗。Objective To observe the clinical effect of acupoint electric stimulation combined with paravertebral nerveblock in the treatment of acute herpes zoster neuralgia.Methods Totally 60 patients with acute herpes zoster(Hz)whomet the diagnostic criteria and inclusion criteria were randomly divided into treatment group and control group with 30 cases in each group.The treatment group was treated with acupoint electrical stimulation combined with paravertebralnerve block,while the control group was treated with paravertebral nerve block.The Jiaji point and Ashi point in thecorresponding segment of the pain area were selected for acupoint electrical stimulation.The visual analog scale(VAS)and Hamilton Anxiety Scale(HAMA)were observed before treatment(T_(0)),on the first day(T_(1)),on the seventh day(T_(2)),and on the fourteenth day(T_(3))to evaluate the clinical efficacy of the two groups.Results Compared with T_(0),the VASscores of the two groups decreased at T_(1),T_(2) and T_(3)(P<0.05).On T_(2) and T_(3),VAS score of treatment group was lower thanthat of control group(P<0.05).After treatment,VAS scores of the two groups were lower,and the treatment group wasbetter than the control group.Compared with T_(0),HAMA score of T_(2),T_(2) and T_(3) was statistically significant(P<0.05).OnT_(2) and T_(3),HAMA score of treatment group was lower than that of control group(P<0.05).The HAMA score of the twogroups decreased after treatment,and the treatment group was better than the control group.Conclusion Thetherapeutic effect of acupoint electric stimulation combined with paravertebral nerve block in the treatment of acuteherpes zoster neuralgia is definite,and it is better than that of paravertebral nerve block only in relieving pain,improvingpatients′anxiety state.
