摘　　要：目的探讨氰基丙烯酸酯栓塞术(CAE)联合泡沫硬化剂治疗下肢静脉曲张的临床疗效。方法收集2020年9月至2021年3月甘肃省人民医院收治40例下肢静脉曲张的患者临床资料。根据治疗方式不同分为射频消融术组(RFA组,n=25)和CAE组(n=15)。比较两组患者主要观察指标(术后1、3个月大隐静脉主干的闭合率);次要观察指标[术后3个月并发症发生情况;术前、术后3个月静脉临床严重程度评分(VCSS),视觉模拟评分量表(VAS),阿伯丁静脉曲张问卷(AVVQ)评分]。结果术后1、3个月,两组患者大隐静脉主干闭合率比较,差异无统计学意义(P>0.05);术后3个月,两组患者的VCSS、AVVQ、VAS评分均低于术前;CAE组患者的并发症发生率低于RFA组患者,差异有统计学意义(P<0.05)。CAE组2例患者出现硬索条状肿块。结论CAE联合泡沫硬化剂治疗下肢静脉曲张在短期疗效中提供了良好的闭合率,有效降低并发症发生率。Objective To investigate the clinical effect of cyanoacrylate embolization(CAE)combined with foam sclerotherapy in the treatment of varicose veins of lower limbs.Method Clinical data of 40 patients with varicose veins of lower limbs admitted to the Gansu Provincial People’s Hospital from September 2020 to March 2021 were collected.According to different treatment methods,all patients were divided into radiofrequency ablation group(RFA group,n=25)and CAE group(n=15).The primary outcome mesures(the closure rate of the main saphenous vein 1 and 3 months after surgery)and secondary outcome measures[complication incidence 3 months after surgery,venous clinical severity score(VCSS),visual analog scale(VAS),Aberdeen varicose vein questionnaire(AVVQ)score before and 3 months after surgery]were compared.Result One and three months after surgery,there was no significant difference in the closure rate of the main saphenous vein between the two groups(P>0.05);3 months after surgery,VCSS,AVVQ and VAS scores in two groups were all lower than those before surgery;the incidence rate of complications in CAE group was lower than that in RFA group(P<0.05).Two patients in the CAE group developed cord-like masses.Conclusion CAE combined with foam sclerosing agent in the treatment of lower limb varicose veins provides a good closure rate and a low complication rate in short term.
