摘　　要：目的探讨多种介入技术联合治疗下肢动脉硬化闭塞症合并急性血栓形成(ASOCAT)的临床疗效。方法收集2018年1月至2020年10月烟台市烟台山医院收治的42例ASOCAT患者的临床资料,评估多种介入技术联合治疗的安全性和有效性。结果41例患者动脉成功开通,手术成功率为97.62%(41/42);1例患者血栓未能清除,经药物保守治疗后好转出院。围手术期无1例患者发生截肢或死亡。随访期间,下肢动脉血栓复发2例,其中1例患者经再次介入手术好转出院,另1例患者予以截肢。结论个体化采取多种介入技术联合治疗下肢ASOCAT可以快速清除血栓,减少溶栓药物使用量,降低支架植入率,值得临床推广。Objective To investigate the clinical efficacy of combination of multiple endovascular techniques in the treatment of arteriosclerosis obliterans complicated with acute thrombosis(ASOCAT)of lower extremities.Method The clinical data of 42 patients with ASOCAT admitted in Yantaishan Hospital from January 2018 to October 2020 was collected to evaluate the safety and efficacy of combination of multiple endovascular techniques.Result The artery recanalization was successfully performed among 41 patients,and the success rate was 97.62%(41/42).Thrombus removal failed for one patient,who was discharged after conservative medication.There were no amputation or death in the perioperative period.During the follow-up,there were 2 cases with lower extremity artery thrombosis recurrence,in which 1 case was discharged with relevant conditions improved after second intervention,and 1 case was amputated.Conclusion Individualized combination of multiple endovascular techniques for treating ASOCAT can remove the thrombus rapidly,reduce the use of thrombolytic drugs and the stenting rate,which is worthy of clinical application.
