基于古代医案分析中医治疗中风失语用药规律

摘　　要：目的:通过文献数据挖掘技术分析古代医案治疗中风失语用药规律。方法:收集第5版《中华医典》电子书医案部分记载中风失语处方的中药,建立数据矩阵信息库,分析处方出现的高频核心药物,然后采用Apriori算法和系统聚类,通过SPSS Modeler 18.0和SPSS Statistics 22.0软件对处方数据库中高频核心药物进行关联分析和聚类分析,分析医案中治疗中风失语的用药规律。结果:处方数据信息提取后纳入治疗中风失语方剂117首,涉及中药共计194味,中药的使用总频次为1 125次,高频核心中药26味。按照中药性味和功能进行统计分析,发现古代医案治疗中风失语多选用化痰止咳平喘药、平肝熄风药、补气药;高频核心药物的寒热温凉药性分析以温性药物为首位,寒性药物次之,药性寒热并用;药味分析以甘味、苦味和辛味为主。关联分析得到天竺黄+钩藤,枳实+半夏,半夏+天竺黄+钩藤等为治疗中风失语核心药物组合,分析高频核心药物网络结得出解语丹、羚羊角散和八珍汤是医案记载治疗中风失语核心方剂。结论:古代医家治疗中风失语用药规律以化痰开窍、清热散瘀、益气活血法为主,可供现代临床治疗中风失语作为参考。Objective:Analyze the rule of medicine used in ancient medical records for the treatment of aphasia due to apoplexy through literature data mining technology.Methods:Collect the Chinese medicines in the medical records section of the 5 th edition of the "Chinese Medical Dictionary" that record stroke aphasia prescriptions,establish a data matrix information database,analyze the high-frequency core drugs appearing in the prescriptions,and then use the Apriori algorithm and systematic clustering to pass SPSS Modeler 18.0 Use SPSS Statistics 22.0 software to conduct correlation analysis and cluster analysis of high-frequency core drugs in the prescription database,and analyze the medication rules in the treatment of stroke aphasia in medical records.Results:After the prescription data information was extracted,117 prescriptions for the treatment of apoplexy were included,involving a total of 194 Chinese medicines.The total frequency of Chinese medicines was 1125 times,and the high-frequency core Chinese medicines were 26.According to the performance theory of the properties and functions of traditional Chinese medicines,it was found that the ancient medical records used phlegm-relieving and cough-relieving anti-asthmatic drugs,liver-eliminating wind-eliminating drugs,and qi-tonicing drugs in the treatment of stroke aphasia;Warm drugs are the first,followed by cold drugs,and the combination of cold and heat is used.The taste analysis of drugs is mainly sweet,bitter and pungent.Correlation analysis showed that Tianzhuhuang(Concretio Silicea Bambusae)+Uncaria(uncaria rhynchophylla),Zhishi(immature bitter orange)+Banxia(Pinellia);Banxia(Pinellia)+Tianzhuhuang(Concretio Silicea Bambusae)+Gouteng(Uncaria)are the core drug combinations for the treatment of stroke aphasia,and the high-frequency core drug network is analyzed to obtain Jieyu Pill,Lingyangjiao Powder and Bazhen Decoction are the core prescription for the treatment of aphasia due to stroke.Conclusion:After data mining and analysis,the ancient medical doctors us
