摘 要:目的观察易黄汤作用后高危型HPV感染患者阴道微生态及阴道局部免疫因子IL-2、IL-6和IL-17的变化,探讨易黄汤治疗HR-HPV的可能机制。方法选择2018年1月至2019年6月就诊我院门诊高危型HPV阳性、TCT正常的脾虚湿热型患者56例,予易黄汤治疗3个月,停药3个月后比较治疗前后阴道微生态及阴道灌洗液中白细胞介素2(IL-2)、IL-6、IL-17变化。结果治疗后阴道正常菌群构成比、正常菌群密集度、正常菌群多样性及正常Nugent评分占比分别为67.86%、57.14%、53.57%及62.5%,均高于治疗前(P<0.05);pH值构成比治疗前后的差异无统计学意义(P>0.05)。IL-6、IL-17水平治疗后低于治疗前;IL-2水平治疗后高于治疗前,三种免疫因子治疗前后差异有统计学意义(P<0.05)。结论易黄汤可能通过增强阴道局部免疫,改善阴道微生态,从而促进HR-HPV清除。OBJECTIVE To observe vaginal microecology and local cervical cytokiness IL-2,IL-6 and IL-17 of patients with HR-HPV infection before and after using Yihuang decoction,in order to explore the possibillity of the immunomodulatory mechanism.METHODS A total of 56 patients with hrHPV positive and TCTNormal of spleen deficient and Damp heat type admitted to our hospital from Jan 2018 to Jun 2019 were selected as study group.These patients were treated by Yihuang decoction for three months.Three months after treatment ended,vaginal microecology and the levels of IL-2,IL-6 and IL-17 were tested between before and after treatment.RESULTS The proportions of normal vaginal flora,the normal density,the diversity of the bacteria and normal Nugent score after treatment were 67.86%、57.14%、53.57%and 62.5%,respectively,significantly higher than those after treatment.The level of IL-6、IL-17 before treatmnet were lower than those after treatment,while the level of IL-2 was higher than that after treatment.The difference in three cytokiness was statistically significant(P<0.05).CONCLUSION Yihuang decoction could remove the hrHPV,its mechanism may be related to regulating local cervical immunity and improving vaginal microecology.
