某院945例万古霉素临床应用分析和合理性评价

摘　　要：目的分析万古霉素临床应用情况,为临床合理有效地使用万古霉素提供依据。方法依据制订的合理用药评估标准,回顾性调查某三甲医院2018年1月1日至2018年12月31日之间接受万古霉素治疗住院患者的临床应用情况,进行合理用药评价。结果2018年住院患者中使用万古霉素的病例为945例,年龄为16天至90岁,来自27个科室,血液科患者占47.1%,最常见的感染部位为呼吸系统感染,284例检测出革兰氏阳性菌,DUI=0.83,CDUI=0.93,治疗有效率为69.3%,不良反应发生率为14.2%,用药不合理率为8.0%。结论该院万古霉素使用科室分布广泛,普遍存在联合用药的情况,临床应用基本合理,但使用过程中仍存在不适宜现象,建议加强监管。OBJECTIVE Analyze the clinical application of vancomycin,aiming to provide references for the rational and effective use of vancomycin.METHODS According to the assessment standard,the clinical records of inpatients treated with vancomycin in a hospital during January 1,2018 to December 31,2018 were retrospectively investigated,and the rational drug use was evaluated.RESULTS 945 cases were derived from 27 departments,involving patients aged from 16 days to 90 years old,mainly from the department of hematology(47.1%)and with respiratory system infection being the most common infection site.Gram positive bacteria were detected in 284 cases.DUI=0.83,CDUI=0.93,the treatment effective rate was 69.3%,the incidence of adverse drug reaction was 14.2%,and 8.0%of cases were not rational.CONCLUSION Vancomycin was widely used throughout the departments in this hospital,and there was a general situation of combined drug use.The clinical application of vancomycin was generally rational but there were still inappropriate phenomena in the clinical use process.The rational use of vancomycin should be strengthened in the regulation.
