杞菊地黄丸联合雷珠单抗治疗糖尿病视网膜病变患者临床效果及对血清PDGF、BMP-2影响

摘　　要：目的观察杞菊地黄丸联合雷珠单抗治疗糖尿病视网膜病变患者临床效果及对血清血小板源性生长因子(PDGF)、骨形成蛋白-2(BMP-2)影响。方法选取2018年10月—2020年10月收治的糖尿病视网膜病变74例,根据治疗方法不同将其分为观察组和对照组两组各37例。观察组给予杞菊地黄丸联合雷珠单抗治疗,对照组给予雷珠单抗治疗。比较两组治疗后临床效果,治疗前后最佳矫正视力、黄斑中心视网膜厚度、血液流变学指标、氧化应激指标、血清PDGF、血清BMP-2、生活质量,以及治疗期间不良反应。结果治疗后,观察组总有效率高于对照组(P<0.05)。治疗后,两组最佳矫正视力、总抗氧化能力和低视力者生活质量量表(CLVQOL)评分高于治疗前,黄斑中心视网膜厚度、血液流变学指标、丙二醛和血清PDGF、BMP-2低于治疗前;观察组最佳矫正视力、总抗氧化能力和CLVQOL评分高于对照组,黄斑中心视网膜厚度、血液流变学指标、丙二醛和血清PDGF、BMP-2低于对照组(P<0.01)。治疗期间,两组不良反应总发生率比较差异无统计学意义(P>0.05)。结论杞菊地黄丸联合雷珠单抗治疗糖尿病视网膜病变患者可改善视力和血液流变学指标,降低黄斑中心视网膜厚度和血清PDGF、BMP-2,减轻氧化应激反应,提高生活质量,且安全性好。Objective To observe clinical effectiveness of Qiju Dihuang Pills combined with Ranibizumab in treatment of patients with diabetic retinopathy and its effects on serum platelet derived growth factor(PDGF)and bone morphogenetic protein-2(BMP-2).Methods A total of 74 patients with diabetic retinopathy admitted between October 2018 and October 2020 were selected and divided into observation group(n=37)and control group(n=37)according to different therapeutic methods.Observation group was treated with Qiju Dihuang Pills combined with Ranibizumab,while control group was treated with Ranibizumab.Clinical effect after treatment,values of the best corrected visual acuity(BCVA),foveomacular retinal thickness,hemorheological indicators,oxidative stress indicators,serum levels of PDGF and BMP-2,quality of life before and after treatment,and incidence rate of adverse reactions during the treatment were compared between two groups.Results After treatment,the total effective rate in observation group was significantly higher than that in control group(P<0.05).After treatment,values of BCVA,total antioxidant capacity and Chinese-version low vision quality of life questionnaire(CLVQOL)were significantly higher,while values of foveomacular retinal thickness,hemorheological indexes,malondialdehyde,serum levels of PDGF and BMP-2 were significantly lower than those before treatment in two groups;in observation group,values of BCVA,total antioxidant capacity and CLVQOL score were significantly higher,while values of foveomacular retinal thickness,hemorheological indexes,malondialdehyde,and serum levels of PDGF and BMP-2 were significantly lower than those in control group(P<0.01).There was no significant difference in the total incidence rate of adverse reactions during the treatment between two groups(P>0.05).Conclusion Qiju Dihuang pills combined with Ranibizumab in treatment of patients with diabetic retinopathy may improve visual acuity and hemorheological indexes,reduce foveomacular retinal thickness and serum levels of PDGF and
