摘　　要：目的:探讨C5a在痛风性关节炎患者中的表达及意义。方法:纳入37例痛风性关节炎(GA)及28例膝骨关节炎(OA)患者,收集所有患者的相关临床资料,另纳入30例健康体检者作为正常对照组(HC)。采用ELISA法检测GA组、OA组及HC组血清C5a浓度,GA组、OA组关节液C5a浓度以及尿酸单钠晶体体外诱导健康人血清生成C5a的浓度。结果:血清C5a浓度在GA组明显高于HC组和OA组,GA组关节液中C5a浓度显著高于配对血清中的C5a浓度和OA组关节液中的C5a浓度;血清C5a浓度与患者受累关节的压痛和肿胀程度呈显著正相关,血清C5a浓度与CRP浓度呈正相关,关节液中C5a浓度与关节液白细胞计数呈现显著相关性,关节液中尿酸盐晶体负荷与关节液C5a浓度密切相关;③尿酸单钠晶体以剂量依赖性方式促进C5a生成。结论:尿酸盐晶体可诱发C5a的生成,血清C5a浓度可以作为判断GA炎症程度的指标。Objective:To investigate the significance of allergic toxin C5a in patients with gouty arthritis.Methods:37 pa-tients with gouty arthritis(GA)and 28 patients with knee osteoarthritis(OA)were included.The relevant clinical data of all patients were collected,and 30 healthy subjects were included as healthy control group(HC).ELISA was used to detect the serum C5a concen-tration of GA,OA and HC group,the C5a concentration of synovial fluid of GA and OA,the serum concentration of C5a induced by monosodium urate crystals in healthy controls in vitro.Results:Serum C5a concentration was significantly higher in GA patients than in HC and OA patients;C5a concentration in the synovial fluid of GA patients was significantly higher than in matched serum and in the synovial fluid of OA patients.The serum C5a concentration was significantly positively correlated with the tenderness and swelling of the affected joints.Serum C5a concentration was positively correlated with CRP concentration,while the concentration of C5a in synovial fluid was significantly correlated with the white blood cell count of synovial fluid.The concentration of urate crystals was closely correlated to the concentration of C5a in synovial fluid.③Monosodium urate crystals promote C5a production in dose-de-pendent manner.Conclusion:Urate crystals can induce the production of C5a,which can be used as an indicator to judge the degree of GA inflammation.
