白银市丙型肝炎诊断及数据质量核查分析

摘　　要：目的了解甘肃省白银市医疗机构现行丙型肝炎诊断标准应用现状,促进丙型肝炎病例诊断和报告的规范性。方法2020年在具备血清抗-HCV检测能力的所有医疗机构中,选择抗-HCV阳性的全部病例,按照现行丙型肝炎诊断标准判定后,复核病例相关信息与传染病报告信息管理系统数据。结果丙型肝炎病例漏报率为48.41%,错误报告率为34.32%,临床诊断病例漏报率(60.26%,χ^(2)=11.500)和错误报告率(41.03%,χ^(2)=7.586)均高于确诊病例(29.17%和19.23%,P<0.05);诊断分类正确率为36.92%,急慢性分类正确率为23.08%。结论白银市医疗机构丙型肝炎病例漏报率及错误报告率较高,诊断分类、急慢性分类正确率较低;建议加强全市丙型肝炎诊断和报告培训及督导,提高工作质量。Objective To understand the application status of the standard of hepatitis C in the medical institutions in Baiyin city,and promote the formalization of the diagnosis and report of the cases with hepatitis C.Methods All the cases of antiHCV positive were selected from the medical institutions with the detectability of anti-HCV in serum,and diagnosed according to the current standard of diagnosis of hepatitis C in 2020.The relevant information of the cases was reviewed with the data from the infectious disease report information management system.Results The rate was 48.41%of missing report,and 34.32%of false report.The missing report rate(60.26%,χ^(2)=11.500)and the false report rate(41.03%,χ^(2)=7.586)of the clinically diagnosed cases were both higher than those of the confirmed cases(29.17%and 19.23%,P<0.05).The correct rate was 36.92%of diagnosis classification,and 23.08%of the acute and chronic classification.Conclusions The missing report rate and false report rate are high,and the correct rates of diagnosis classification and acute and chronic classification are low in the cases of hepatitis C among the medical institutions in Baiyin city.The training and supervise of the training about diagnosis and report on hepatitis C are suggested to be strengthened for improving the working quality in the city.
