宫外孕破裂和卵巢黄体破裂阴道彩色多普勒超声检查在血流量特征血流动力参数和血流频谱的差异

摘　　要：目的探讨宫外孕破裂和卵巢黄体破裂阴道彩色多普勒超声检查在血流量特征、血流动力参数和血流频谱的差异。方法随机选取我院2019年5月至2021年5月间经病理证实的45例宫外孕破裂和45例卵巢黄体破裂患者,对其均予以经阴道彩色多普勒超声检查,测定并比较2组患者血流量特征、血流动力参数及血流频谱情况。结果经阴道彩色多普勒超声检查用于宫外孕破裂、卵巢黄体破裂的诊断成功率分别为96%(43/45)、91%(41/45),组间比较,差异无统计学意义(P>0.05);经阴道彩色多普勒超声下宫外孕破裂患者血流信号主要为条状、点状,血流频谱为多样,多为高阻力型。而卵巢黄体破裂患者则形态不同,血流型号较少,血流频谱多为低阻力型,部分有盆腔积液,组间比较,差异有统计学意义(P<0.05)。结论宫外孕破裂和卵巢黄体破裂在阴道彩色多普勒超声下有明显差异,阴道彩色多普勒超声对宫外孕破裂和卵巢黄体破裂有鉴别诊断价值。Objective To explore the differences in blood flow characteristics,hemodynamic parameters and blood flow spectrum between rupture of ectopic pregnancy and rupture of corpus luteum by vaginal color Doppler ultrasound.Methods A total of 45 patients with rupture of ectopic pregnancy and 45 patients with rupture of corpus luteum pathologically confirmed in the hospital were randomly enrolled between May 2019 and May 2021.All underwent examination of transvaginal color Doppler ultrasound.The blood flow characteristics,hemodynamic parameters and blood flow spectrum were compared between the two groups.Results The diagnostic success rates of transvaginal color Doppler ultrasound for rupture of ectopic pregnancy and rupture of corpus luteum were 96%(43/45)and 91%(41/45),respectively,and the difference was not statistically significant(P>0.05).In patients with rupture of ectopic pregnancy,transvaginal color Doppler ultrasound showed that blood flow signals were mainly striped and punctate.The blood flow spectrum was diverse,mainly on high-resistance type.In patients with rupture of corpus luteum,morphology was different.There were fewer blood flow signals.Most blood flow spectrum was low-resistance type,and some was accompanied with pelvic effusion,and the differences between the two kinds of patients were statistically significant(P<0.05).Conclusion There are significant differences between rupture of ectopic pregnancy and rupture of corpus luteum under vaginal color Doppler ultrasound.Therefore,vaginal color Doppler ultrasound is of significant differential diagnostic value for rupture of ectopic pregnancy and rupture of corpus luteum.
