摘　　要：目的:了解雅安市农村养老院老人甲状腺功能现状。方法:在雅安市农村养老院中随机抽取291个老人为研究对象,检测所有老人的血清促甲状腺素(TSH)水平和血糖水平。按照年龄分为A组(n=126,≥70岁)和B组(n=165,<70岁),比较两组TSH高值阳性检出率;按照性别分为男性组(n=271)和女性组(n=20),比较两组TSH高值阳性检出率;按照甲状腺功能状况分为C组(n=221,甲状腺功能正常)和D组(n=70,甲状腺功能异常),比较两组异常血糖的阳性检出率。结果:本次共检测出亚临床甲减66例,占比22.68%(66/291),甲亢1例,占比0.34%(1/291),亚临床甲亢3例,占比1.03%(3/291);A组TSH高值阳性检出率为30.16%(38/126),明显高于B组的16.97%(28/165),差异有统计学意义(P<0.05);男性组TSH高值阳性检出率为22.88%(62/271),与女性组的20.00%(4/20)比较,差异无统计学意义(P>0.05);C组异常血糖的阳性检出率为15.84%(35/221),与D组的17.14%(12/70)比较,差异无统计学意义(P>0.05)。结论:雅安市农村养老院老人亚临床甲减检出率达22.68%,建议应重视老人甲状腺功能健康问题。Objective:To investigate the current status of thyroid function of the elderly people in rural nursing homes in Ya’an city.Methods:291 elderly people were randomly selected from the rural nursing homes in Ya’an city as the research objects.Detect the thyroid stimulating hormone(TSH)levels and the blood glucose levels in the serum samples of all the elderly people.They were divided into group A(n=126,≥70 years old)and group B(n=165,<70 years old)according to their ages,and the detection rate of the abnormally elevated TSH level was compared between the two groups.They were also divided into male group(n=271)and female group(n=20)by gender,and the detection rate of the abnormally elevated TSH level was compared between the two groups.Further,according to the thyroid function status,they were divided into group C(n=221,normal thyroid function)and group D(n=70,abnormal thyroid function),and the positive detection rate of abnormal blood glucose was compared between the two groups.Results:Among these elderly people,66 cases of subclinical hypothyroidism were detected this time,accounting for 22.68%(66/291);1 case had hyperthyroidism,accounting for 0.34%(1/291);3 cases suffered from subclinical hyperthyroidism,accounting for 1.03%(3/291).The detection rate of the abnormally elevated TSH level in group A was 30.16%(38/126),which was significantly higher than 16.97%(28/165)in group B,and the difference was statistically significant(P<0.05).The positive detection rate of the abnormally elevated TSH level in the male group was 22.88%(62/271),compared with 20.00%(4/20)in the female group,the difference was not statistically significant(P>0.05).Further,the positive detection rate of the abnormal blood glucose level in group C was 15.84%(35/221),compared with 17.14%(12/70)in group D,the difference was not statistically significant(P>0.05).Conclusions:The detection rate of subclinical hypothyroidism for the elderly people in the rural nursing homes in Ya’an city is up to 22.68%.It is recommended that attention should be
