皮肤牵张闭合器联合切开减压一期治疗高原地区骨筋膜室综合征的疗效观察

摘　　要：目的分析皮肤牵张闭合器联合切开减压一期治疗高原地区骨筋膜室综合征的临床疗效。方法将西宁市第二人民医院2018年1月-2021年1月收治的共96例骨筋膜室综合征患者作为研究对象,采用随机数字表法分为观察组和对照组,各48例。对照组仅通过切开减压一期治疗,观察组在对照组基础上联合使用皮肤牵张闭合器,比较两组换药次数、肿胀完全消失时间、抗生素使用时间、创面封闭时间、住院时间、膝关节Lysholm评分、VAS评分以及感染发生率。结果观察组换药次数、肿胀完全消失时间、抗生素使用时间、创面封闭时间以及住院时间均低于对照组,差异有统计学意义(P<0.05);治疗后,两组膝关节Lysholm评分均高于治疗前,VAS评分均低于治疗前,且观察组Lysholm评分均高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05);观察组感染发生率为4.17%,低于对照组的22.92%,差异有统计学意义(P<0.05)。结论皮肤牵张闭合器联合切开减压一期治疗高原地区骨筋膜室综合征具有良好的临床疗效,能够有效促进患者的康复,缓解患者疼痛感,改善膝关节功能,且有利于避免感染的发生。Objective To analyze the clinical efficacy of skin stretch closure device combined withone-stage incision decompression in the treatment of osteofascial compartment syndrome in plateau area.Methods A total of 96 patients with osteofascial compartment syndrome admitted to Xining Second People's Hospital from January 2018 to January 2021 were randomly divided into observation group and control group,with 48 cases in each group.The control group was only treated with one-stage incision and decompression,and the observation group was treated with skin tension closure device on the basis of the control group.The frequency of dressing change,the time of complete disappearance of swelling,the time of antibiotic use,the time of wound closure,the time of hospitalization,Lysholm score of knee joint,VAS score and the incidence of infection were compared between the two groups.Results The frequency of dressing change,the time of complete disappearance of swelling,the time of antibiotic use,the time of wound closure and the time of hospitalization in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,Lysholm score of knee joints in the two groups was higher than those before treatment,and VAS score was lower than those before treatment,and Lysholm score in the observation group was higher than those in the control group,and VAS score was lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of infection in the observation group was 4.17%,which was lower than 22.92%in the control group,and the difference was statistically significant(P<0.05).Conclusion The skin stretch closure device combined withone-stage incision decompression has a good clinical effect in the treatment of osteofascial compartment syndrome in plateau area,which can effectively effectively promote the rehabilitation of patients,relieve pain,improve knee function,and help to avoid infection.
