血小板分布宽度与冠心病心力衰竭的相关性探讨

摘　　要：目的 探讨血小板分布宽度与冠心病心力衰竭的相关性。方法 150例冠心病患者,根据心力衰竭程度[N末端B型钠尿肽前体(NT-ProBNP)水平]分为重度组(NT-ProBNP水平>3000 ng/L)、轻度组(NT-ProBNP水平300~3000 ng/L)及对照组(NT-ProBNP<300 ng/L),各50例。检测并比较三组患者治疗前后的血小板分布宽度,分析血小板分布宽度与NT-ProBNP水平的相关性。结果 治疗前,重度组血小板分布宽度为(0.149±0.017),轻度组血小板分布宽度为(0.141±0.022),对照组血小板分布宽度为(0.131±0.019);治疗后,重度组血小板分布宽度为(0.139±0.014),轻度组血小板分布宽度为(0.133±0.013),对照组血小板分布宽度为(0.127±0.015)。治疗前,重度组血小板分布宽度高于轻度组及对照组,轻度组血小板分布宽度高于对照组,差异具有统计学意义(P<0.05)。治疗后,三组血小板分布宽度均明显低于治疗前,差异具有统计学意义(P<0.05);重度组血小板分布宽度高于轻度组及对照组,轻度组高于对照组,差异具有统计学意义(P<0.05)。血小板分布宽度与NT-ProBNP水平呈正相关(r=0.517,P<0.05)。结论 冠心病心力衰竭患者随疾病严重程度升高,血小板分布宽度随之升高,二者呈正相关,因此血小板分布宽度测定对判断冠心病心力衰竭病情具有辅助作用。Objective To discuss the correlation between platelet distribution width and coronary heart disease and heart failure.Methods A total of 150 patients with coronary artery disease were divided into severe group(NT-ProBNP>3000 ng/L),mild group(NT-ProBNP 300-3000 ng/L)and control group(NT-ProBNP<300 ng/L)according to the degree of heart failure[N-terminal pro-B-type natriuretic peptide(NT-ProBNP)level],with 50 cases in each group.The platelet distribution widths of the three groups were measured and compared before and after treatment,and the correlation between platelet distribution widths and NT-ProBNP levels was analyzed.Results Before treatment,the platelet distribution width of the severe group was(0.149±0.017),the platelet distribution width of the mild group was(0.141±0.022),and the platelet distribution width of the control group was(0.131±0.019);after treatment,the platelet distribution width of the severe group was(0.139±0.014),the platelet distribution width of the mild group was(0.133±0.013),and the platelet distribution width of the control group was(0.127±0.015).Before treatment,the platelet distribution width of the severe group was higher than that of the mild group and the control group,and the platelet distribution width of the mild group was higher than that of the control group.All the differences were statistically significant(P<0.05).After treatment,the platelet distribution width in the three groups was significantly lower than those before treatment,and the difference was statistically significant(P<0.05);the platelet distribution width of the severe group was higher than that of the mild group and the control group,and the mild group was higher than the control group;all the differences were statistically significant(P<0.05).Platelet distribution width was positively correlated with NT-ProBNP level(r=0.517,P<0.05).Conclusion The platelet distribution width increases with the increase of disease severity in coronary heart disease patients with heart failure,and the two are positively co
