低分子量肝素钠在高龄产妇剖宫产术后下肢深静脉血栓预防中的价值分析

摘　　要：目的 探讨低分子量肝素钠在高龄产妇剖宫产术后预防下肢深静脉血栓中的临床效果。方法 60例行剖宫产的高龄产妇,随机分为对照组及研究组,每组30例。对照组进行常规干预,研究组在对照组的基础上联合低分子量肝素钠进行干预。对比两组产妇干预前后的凝血指标[凝血酶原时间(PT)、凝血酶时间(TT)、血小板计数(PLT)]、下肢深静脉血栓发生情况、满意度。结果 干预后,两组产妇TT短于本组干预前、PLT低于本组干预前,且研究组产妇TT(14.6±1.7)s短于对照组的(15.8±1.3)s、PLT(153.2±21.5)×109/L低于对照组的(195.6±19.6)×109/L,差异具有统计学意义(P<0.05)。研究组产妇的下肢深静脉血栓发生率为0,低于对照组的13.3%,差异具有统计学意义(P<0.05)。研究组产妇的满意度为96.7%,高于对照组的73.3%,差异具有统计学意义(P<0.05)。结论 采用低分子量肝素钠对高龄剖宫产产妇进行干预治疗,可有效降低下肢深静脉血栓的发生率,避免血液长期处于高凝状态,具有较高的治疗安全性。Objective To discuss the clinical effect of low-molecular-weight heparin sodium in the prevention of deep vein thrombosis of lower extremities after cesarean section in women of advanced maternal age.Methods A total of 60 women of advanced maternal age undergoing cesarean section were randomly divided into control group and research group,with 30 cases in each group.The control group received conventional intervention,and the research group received low-molecular-weight heparin sodium on the basis of the control group.Both groups were compared in terms of ccoagulation indexes[prothrombin time(PT),thrombin time(TT),platelet count(PLT)]before and after the intervention,occurrence of deep vein thrombosis of the lower extremities,and satisfaction.Results After the intervention,TT in both groups was shorter than that before the intervention,and PLT was lower than that before the intervention;TT(14.6±1.7)s of the research group was shorter than(15.8±1.3)s of the control group,and PLT(153.2±21.5)×109/L was lower than(195.6±19.6)×109/L of the control group;all the differences were statistically significant(P<0.05).The incidence of deep vein thrombosis of the lower extremities in the research group was 0,which was lower than 13.3%in the control group,and the difference was statistically significant(P<0.05).The maternal satisfaction rate of the research group was 96.7%,which was higher than 73.3%of the control group,and the difference was statistically significant(P<0.05).Conclusion The use of low-molecular-weight heparin sodium for of women of advanced maternal age with cesarean section can effectively reduce the incidence of venous thrombosis of lower extremities and avoid long-term hypercoagulation of blood,which has a high therapeutic safety.
